Procedure

1. Declaration of the occupation risk must be filled with the Caisse Nationale de Sécurité Sociale (CNSS) office near you within 48 hours of the risk occurring.
2. Fill out four copies of declaration form which can be obtain at any CNSS branch office or download it via the CNSS website portal or click the link below to download (PROFESSIONAL RISK DECLARATION FORM)
3. The declaration forms should then be submit as follows:
   • First Copy must be sent by registered letter with acknowledgment of receipt or hand delivered to the National Social Security Fund, within 48 hours of the risk.
   • Second Copy must be sent to the relevant Labor Inspector.
   • Third Copy must be sent to the Doctor dealing with.
   • Fourth Copy must be kept by the Employer for records
4. The declaration form submit to the CNSS will be verified and processed, If the authorized official is satisfied that all the documents are in order you will be notified of the outcome and be given further instruction.

Need for the Document

• The benefits are issued to any insured person who, as a result of occupation risk/accident of professional origin, is no longer able to work due to their physical conditions are entitled to the benefit.

• Coverage of hospital costs due to accident or occupational disease;
• External costs;
• Evacuations.
