ProcedureEdit

Apply Online

  1. To Replace Pension Allowance Card, the applicant has to make the application in Ethiopia E – service portal or tap the following link
  2. The applicant has to complete the online application form for Replace Pension Allowance Card or tap the following link
  3. The applicant must read through the instruction and click on the “I agree and continue” icon.
  4. The applicant has to login with their Email id or phone number and password. (If the applicant doesn’t have an account the applicant has to click on “Register” and register for new account.
  5. After logging in the applicant must fill in all the necessary details in the application form.
  6. Make sure that the applicant has to upload all the necessary documents (photocopies and originals) that are mentioned in “Required Documents” in the section of this page.
  7. The applicant has to pay the service fee as per the authority request.
  8. Review and submit the application. For successful submission the applicant will gets the notification regarding application to the register email id or mobile number.


Note:

  • After you Submit your Service Request Application, you will be provided with a unique Tracking Number and Service provider branch that will process your service request. This information will also be sent to your email address you provided at registration time. With your tracking number, you can follow up the progress of your application on Track My Application Section. You can only apply for the Same Service Request once. Only after the completion of existing Service request, you can reapply to the Service again.
  • For Service requests that requires a physical presence as a part of business process (Example: Payment, hard print document verifications, Document Pickup), you will be asked to reserve appointment time On Appointment Section. Once you complete your reservation, the provider will reserve the time and service counter for you. You can’t change your reservation time, so make sure you will be at reserved service counter before 10 minutes of reservation time.


Apply In Person

  1. To Replace Pension Allowance Card, the applicant has to approach Ministry of Labour and Social affairs link
  2. An application from will be provided to the applicant by the respective officer.
  3. Applicant has to fill in all necessary details in the application form (Incomplete application forms are not accepted).
  4. Make sure that the applicant has all the necessary documents (photocopies and originals) that are mentioned in “Required Documents” in the section of this page.
  5. Applicant should attach the photocopies of the necessary documents (whichever is necessary) along with the application form while submitting.
  6. Submit the application form to the respective officer. The authority will review the application.
  7. Fee for this procedure has to be paid as per the authority request and submit the receipt as a proof of payment.
  8. Upon the successful verification, the authority will issue the replaced pension allowance card within 2 weeks.

  • Entitled beneficiaries or receivers of payment allowance of social security services apply for replacement of pension allowance receiving card and/ or payment control card. If the pension allowance receiving card and/ or payment control card is lost or damaged, the customer can request for replacement of pension allowance receiving card and/ or payment control card by submitting filled service application form. The agency reviews the service request and if requirements are fulfilled, replacement of pension allowance receiving card and/ or payment control card will be provided.
