Oregon - Obtain a Domestic Partnership Certificate

ProcedureEdit

In-Person
  1. Room 205, 800 NE Oregon Street, Portland Monday through Friday between 8:00 a.m. and 4:30 p.m.
  2. Please fill in the application for applying a Domestic Partnership certificate.
  3. Pay a fees of $20.
By Mail
  1. Include an Order form.
  2. Attach a photocopy of your driver's license or official ID.
  3. Send your order to Oregon Vital Records, PO Box 14050, Portland, OR 97293-0050.
  4. Also send a fee of $20, Checks and money orders may be made payable to OHA/Vital Records.
  5. Processing of orders may be delayed by three weeks if payment is by check and the check is for a new bank account with a low check number or is made with a temporary check, the check is from an unrelated third party, or the printed name or address shown on the check has been altered.

By Fax
  1. You may send a fax directly to: (503) 234-8417.
  2. Please include complete vital record information and identification documentation and include your credit card number with expiration date.
  3. Orders are mailed either the same day or the next working day by regular mail unless record problems are discovered.
Speed processing
  1. To speed processing, send your request by any overnight delivery service to Oregon Vital Records, 800 NE Oregon Street, Suite 205, Portland OR 97232.
  2. Orders received by priority mail or overnight delivery are usually sent out by regular mail either the same day or the next working day assuming no record problems are discovered.
  3. If you need the order to be returned by overnight delivery, please enclose a pre-addressed, prepaid overnight delivery envelope.
  4. Processing of orders may be delayed by three weeks if payment is by check and the check is for a new bank account with a low check number or is made with a temporary check, the check is from an unrelated third party, or the printed name or address on the check has been altered. We may not be able to process your request if you are not eligible to order, the record event is so recent that we have not received the record, your order information does not match what is on the record, or you do not have valid ID.

  2. Fees are $32.50.

EligibilityEdit

Access to domestic partnership records filed in the State Vital Records office is restricted for 50 years after the date of the event to family members and their legal representatives, persons or organizations with a personal or property right, government agencies and persons licensed in Oregon under ORS 703.430. When ordering a divorce record, you must state your relationship to the registrant, or your reason for needing the record. If you are a legal representative of a family member, include whom you represent and how they are related to the person named on the record. If you have a personal or property right, explain why you need the record.If you are not eligible to order the record, please include a permission note with the notarized signature of an eligible person.

  • The non-refundable fee is $20 and every additional copy of the same record ordered at the same time is $15. Payment by check, cash or money order ONLY.

The average wait time to receive a record is 15-20 minutes.Expect longer wait times of 30-45 minutes Mondays and Fridays and daily from 11:30 a.m. to 2 p.m
Orders need to be placed atleast by 4:30 p.m. to receive them the same day
