Apply In Person

  1. To Apply for National Skin Cancer Control Program Card( Cartão do Programa Nacional de Controle do Câncer de Pele), the applicant has to approach the Ministério da Saúde (Ministry of Health) link
  2. Schedule regular visits to a dermatologist for professional skin examinations, especially if you have a history of sun exposure or other risk factors.
  3. An application form will be provided to the applicant by the concerned officer.
  4. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
  5. Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
  6. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
  7. Submit all the documents with the completed form to the concerned officer.
  8. There is no fee for this program card.
  9. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
  10. Then the application will be processed further.
  11. Participate in educational programs offered by healthcare facilities and organizations that focus on sun protection, skin cancer awareness, and early detection.
  12. If a dermatologist identifies a suspicious lesion, they might recommend a biopsy for diagnosis.
  13. If skin cancer is diagnosed, the dermatologist will discuss treatment options, which might include surgery, topical treatments, radiation therapy, or other interventions.
  14. Maintain any medical records, diagnostic reports, and treatment plans related to your skin cancer care.
  15. Regular follow-up appointments with your dermatologist are crucial for monitoring your skin health and adjusting the treatment plan if necessary.
  16. This entire process can be completed within a week or as per the timeline advised by the respective department authority.
