Apply In Person

1. To Apply for the National Program for the Prevention and Control of Prostate Cancer card( Cartão do Programa Nacional de Prevenção e Controle do Câncer de Próstata), the applicant has to approach the Ministério da Saúde (Ministry of Health) link
2. Visit a designated healthcare facility that offers health services. Public health clinics and specialized treatment centers are typically involved in Prostate Cancer prevention and treatment.
3. An application form will be provided to the applicant by the concerned officer.
4. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
