Apply In Person

1. To Apply for National Program card for Comprehensive Health Care for the Homeless Population (Cartão do Programa Nacional de Atenção Integral à Saúde da População em Situação de Rua), the applicant has to approach the Local Health Care Facility or Ministério da Saúde (Ministry of Health) link
2. An application form will be provided to the applicant by the concerned officer.
