Write a request with as much as required information with appropriate fees and required documents to the Clerk of Court in county where marriage license was issued.
Required DocumentsEdit
A photocopy of a valid government issued photo ID by either the applicant or an individual attesting for the applicant is required for issuance of certificates.

- State Issued Drivers license
- State Issued Photo Identification card
- US Passport
- Government issued Military ID
- US Bureau of Indian Affairs photo ID
- Tribal photo ID
- Oklahoma gun permit
- Pilot license
- A non US passport and I-94 card (VISA)
- Consular Matricula card + 1 form of secondary ID
NO 3rd Party Vouching (i.e. friend cannot vouch for Mom to get child's certificate) Check allowable applicant list.

- Bank statement with name and current address

- Copy of official police report related to the theft of ID
- Utility bill with a current address
- For any person under the age of 18, an affidavit signed by the parent or legal guardian
- Photo identification card issued by one of the following: ?Oklahoma public, private, or parochial secondary school
- Oklahoma institution of higher education
- Oklahoma technology center school
- Oklahoma employer
- Oklahoma lifetime hunting or fishing license
- Marriage certificate
- Separation or divorce judgment
- Health, life, or home insurance policy issued to the applicant
- Automobile insurance policy or security verification form issued to the applicant
- A valid U.S.D.O.T. health card, as required by 49 C.F.R. Part 391
- An identification document issued by the United States Armed Services; one of the following: ?Military discharge (DD-214), unless specified not to be used for identification
- Military identification card
- Military dependent identification card
- United States Bureau of Indian Affairs identification card or a Oklahoma tribal photo Identification card which identifies the person and includes all the following information: ?color photograph of the person
- full legal name of the person
- birth date of the person
- signature of the person
- signature of person who verifies records
- tribal seal
- Out-of-state driver license, if the issuing state participates in the REAL ID Act of 2005

By Oklahoma state law, Oklahoma Marriage certificates are confidential records and can only be obtained by the following authorized individuals:
- Person Named on the Oklahoma Marriage Certificate
- Mother/Father (or Legal Guardian) of Person Named on the Oklahoma Marriage Certificate
- Grandparents and Great Grandparents of Person Named on the Oklahoma Marriage Certificate
- Husband/Wife of Person Named on the Oklahoma Marriage Certificate
- Adult Son/Daughter of Person Named on the Oklahoma Marriage Certificate
- Legal Representative of an Authorized Person of Person Named on the Oklahoma Marriage Certificate

- Oklahoma Marriage certificates are not maintained by the Oklahoma State Office of Vital Records.
- To obtain an Oklahoma Marriage Certificate you must contact the Clerk of Court in county where license was issued.
- You may order your certified copy either by in person or by mail.
- For fee details contact the Clerk of Court in county where license was issued.
- Enclose a copy of a current legal photo ID
- Person requesting to receive a marriage certificate must sign below
- If submitting by mail, enclose a self-addressed stamped envelope
