Procedure

Apply In Person

1. To Apply for Basic Care Program Card for Homeless People (Cartão do Programa de Atenção Básica às Pessoas em Situação de Rua), the applicant has to approach the Local Health Care Facility or Ministério da Saúde (Ministry of Health) link
2. Based on the evaluation, a healthcare professional will develop a treatment plan that may include vision-related treatments, medications, or assistive devices.
3. An application form will be provided to the applicant by the concerned officer.
4. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)

7. Submit all the documents with the completed form to the concerned officer.

10. Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
11. Once the process gets completed, the applicant gets a call from the office regarding the application.
12. If your application is approved, you will be notified and provided with the Basic Care Program Card, which may entitle you to certain benefits and services for homeless individuals.
13. This entire process can be completed within 2 – 3 weeks or as per the timeline advised by the respective department authority.
