Procedure

How to apply:

1. You should contact the Madirelo Training and Testing Centre or Labour Offices nationwidefor more information on how to file the application.
2. Accurately fill in the application form, which can be obtained from the above offices or click the link below to download.(Transfer of Contract Form)
3. Submit the completed form along with the required supporting documentation (as listed below)for verification and processingat the MTTC office.
4. If theMTTC officeris satisfied with the information obtained applicant Form, the details will be entered into the (MTTC) systemand you will then be notified.

• All citizens who are trainees in an apprenticeship program qualify for this service.

Fees

• There are no costs associated with this service.

Instructions

Transfer of Contract Formhelp to notify MTTC that there is still an agreement between the parties even though the trainee took longerr than contracted to fulfil it. This may be caused by the trainee failing examinations or even illnesses.

• Present employer detail
• New Employer detail
• Declaration by the employer

• The apprenticeship contract transfer allows the trainee to be recognized for completing the apprenticeship program with the Madirelo Training and Testing Centre (MTTC)
