ProcedureEdit

The simplest way to get copies of a death certificate is to order them through the funeral home or mortuary at the time of the death or go to Civil Registry where the death was registered and request a copy of the death certificate. You should present the required documents to the registrar who will search in their database for that particular death entry and if the information is there you will pay the required fee and you will be issued with a copy of death certificate.

One (1) of the following forms of valid photo-ID:

  • Driver license
  • State issued non-driver photo-ID card
  • Passport
  • Bolivian Military issued photo-ID

Two (2) of the following showing the applicant's name and address:

  • Utility or telephone bills
  • Letter from a government agency dated within the last six (6) months

If the applicant is not the spouse, parent, child or sibling of the decedent, a lawful right or claim must be documented. An example of a lawful right or claim would be a death record needed by the applicant to claim a benefit. Documentation would consist of an official letter from the agency verifying that to process the claim they require from the applicant a copy of the requested death record.

  • Gender of the person who died
  • Date of death
  • Name of city or town in which death took place
  • Parental information about the person who died
  • Spouse or partner information of person who died, if applicable

Death certificates are created when someone has passed away.

Death certificates are needed for Social Security, life and health insurance, and to wrap up a deceased ones affairs. These documents are easily obtained.

You can obtain a copy of a death certificate from the Civil Registry in the city or town where the decedent died.
