Procedure

Apply In-Person:

1. To Obtain Proof of Age Card, the applicant has to approach the transport and motoring customer service centre, participating Queensland Government Agency Program Office (QGAP), or a police station that provides licensing services. 2. An application form is available at the following Link
3. The applicant has to get the form in Access Canberra Service Centre and fill-in all the details in the form.
4. Applicant should make sure to attach all the documents that are mentioned under the “Required Documents” section of this page (Both photocopies and originals).
5. Submit the application with the required document to appropriate office.
6. Submitting address is mentioned in “Office Locations & Contacts” of this page.
7. The fee of this procedure is mentioned under the “Fees” section of this page.
8. Proof of age card processed within 14 days.
9. Government of Queensland will issue the certificate and notice the applicant via mail or standard post or email.

Required Information

• QLD driver licence, industry authority, marine licence indicator, APA card or Photo ID card details

Need for the Document

• For Queenslanders who are 15 years of age or older, a photo identity card that serves as age verification is an acceptable alternative to a driver's licence or passport.
