By Mail
  • You may print out, fill and mail (do not email) the birth search application form.

Or

  • You may send a letter with the following information
  1. The birth name of the person on the certificate.
  2. The date of birth.
  3. The city of birth including the county if known.
  4. The mother's full maiden name.
  5. The fathers full name.
  6. Requestor's relationship to the person on the birth certificate.
  7. The purpose for which you are requesting the birth certificate.
  8. Your name and signature.
  9. Your mailing address.
  • If you receive your mail at a Post Office Box, please include a physical street address for our internal files.
  • New Mexico Vital Records, Post Office Box 25767, Albuquerque, NM 87125.
In-Person
  • Applicant must present current government issued picture identification and complete a Search Application. The search fee for a birth certificate is $10.00 per certificate.
  • Counter Service for birth certificates through the Albuquerque Stanford Public Health Office: Monday through Friday from 9:00 am to 4:00 pm and closed 12-1 daily. Please note that on the 1st Tuesday of every month, the counter is closed from 11:00 am to 5:00 pm.
  • The Albuquerque Office is in Stanford Public Health Office at 1111 Stanford NE.

EligibilityEdit

  • State law restricts access to the registrant's Immediate Family members or those who represent tangible proof of legal interest in the requested record.
  • Immediate Family means any of the following: mother, father, sibling, child, current spouse, or maternal or paternal grandparent. Paternal grandparent is eligible if father is listed on the vital record.



FeesEdit

  • The fee for a birth certificate is $10 per certified copy. Please make your certified check or money order payable to "New Mexico Vital Records" in the exact amount.
  • Do not send cash.
