ProcedureEdit

Request a driver history abstract by online For your convenience, you can request your abstract online (Note: credit card convenience fee applies). Visit www.njmvc.gov or http://www.state.nj.us/mvc/Licenses/driver_history_page.htm Follow the Steps: Step 1 - User Authentication

  • Enter User ID Number
  • Enter your driver license number
  • Enter the last four digits of your Social Security number
  • Click "Continue"

Step 2 - Select Type

  • Choose the type of driver history abstract you want
  • There is a $15 non-refundable fee for this transaction (Note: a credit card convenience fee will be applied in order to process each online transaction)
  • Click "Continue"

Step 3 - Non-Certified Driving Record Advisory

  • Read the advisory
  • Click "Continue"

Step 4 - Driver History Credit Card Entry Form

  • Enter "Bill to" information
  • Enter "Payment" information
  • Enter email address in order to receive a confirmation message
  • Click "Continue"

Request a driver history abstract by mail To request a certified copy of a driver history abstract:

  • Complete and sign a Driver History Abstract Request form (DO-21); choose certified complete or a 5-year record
  • Explain in detail the reason for your request
  • Select the permitted use for your request and mark the number on the form
  • Submit a photocopy of your driver license
  • Submit a $15 check or money order payable to NJMVC

EligibilityEdit

The Persons who is eligible to ask Driver History abstract are: Permitted Users of Driver History Record Abstracts There are a number of authorized users who may request a certified driver history record abstract from the Motor Vehicle Commission (MVC). Permitted users include:

  • Attorneys
  • Employers
  • Insurance Companies and Agencies
  • Motorist
  • Municipal Courts, Local and Foreign States Police/Law Enforcement Agencies
  • Other permitted users (if the request provides notarized written consent from the motorist in question or if the users request is permitted under N.J.S.A. 39:2-3.4 (c))



FeesEdit

Obtain a certified Driver History Abstract for $15
Note: MVC accepts American Express card, MasterCard card, Visa card, checks, money orders and cash

Certified, five-year driver history abstracts are also available for purchase at all local motor vehicle agencies and regional service centers. Instructions to apply online

  • Use online application to request a Driver History Abstract

  • Have your credit card (Visa, Master card, Amex, or Discover) and driver license number ready before you begin. You will also need to know your Social Security Number
  • A credit card convenience fee will be applied in order to process each online transaction

Need for the DocumentEdit

MVC keeps all your driving records in a document known as a driver history abstract. A certified driver history record abstract lists all accidents, violations and actions (including, but not limited to suspensions, points credits, fee payments and restorations) incurred by a motorist. It presents the user with an official record of these events.

  • Follow steps described above for abstract purchase
  • On the same form, select document type(s) and fill in the date of each supporting document
  • Include a separate check or money order payable to NJMVC for the total cost of each request type; one check for abstract total and one check for supporting document total
  • For example, submit two checks - $15 check for an abstract and a $30 check for supporting documents - when ordering an abstract, certified order of suspension and a copy of a mailing list

Request certified documents without a driver history abstract by mail

  • To only request certified copies of supporting documents:
  • Complete and sign a Driver History Abstract Request form (DO-21)
  • Select document type(s) and fill in the date of each supporting document

  • It is free to register for My MVC. You will receive your account number through the mail in 7-10 days. We cannot give you your number through the Internet
