Those without acceptable photo identification shall supply a photocopy of two (2) documents listed below. Any document submitted shall be in the name of the individual requesting the record.

  • Copy of income tax return
  • Pay stub (W-2)

Certified copies of New Hampshire death certificates will be issued to the next of kin or to persons with a legal right to the certificate.

InstructionsEdit

  • The New Hampshire Division of Vital Records (New Hampshire Vital Records) issues certified copies of New Hampshire death certificatesfor events which occurred within the State of New Hampshire.
  • New Hampshire vital records are considered to be private and New Hampshire Statute restricts access to them to only those individuals who can demonstrate a "direct and tangible interest" in the record. Certain older records are exempt from these access restrictions. Death records prior to 1911 are available with unrestricted access.
  • You may order your certificate by mail, in person and/or by online (VitalChek)
  • A legible photo-copy of the applicants government issued photo id must be included with this request (i.e. drivers license, non-drivers ID, passport).
  • Please make checks or money orders payable to "Treasurer-State of New Hampshire"
  • While requesting the certified copy of death certificate must mention that with cause of death or without cause of death certificate
  • Also enclose a stamped, self-addressed, business-letter-sized envelope

  • Failure to sign & submit two acceptable documents in place of the required picture identification shall result in the application being rejected & returned to the requester. One of the documents must reflect current physical address.
  • State office has records since 1640. Copies of records may be obtained from State office or from City or Town Clerk in place where event occurred or the license was registered. Recent records (birth since 1983, death since 1990, and marriage since 1989) may be obtained from ANY City or Town running the Vital Records Automated software called NHVRIN.
  • Additional copies ordered at the same time are $10.00 each.
