ProcedureEdit

  1. Get in touch with the Alabama school system or nonpublic school where you were last employed to determine if your original certificate or license remains on file in that office. Note: A duplicate Substitute License will not be issued.
  2. Complete the duplicate request form (a link to download the form is provided under 'Documents to Use' section)
  3. A nonrefundable fee of $30.00 is required for a duplicate certificate. Your payment can be in the form of a money order or cashiers check made payable to the Alabama State Department of Education. You can also pay the fee through the Alabama State Department of Education Educator Certification Online Payment System, with a major credit card - LINK (a $4.00 transaction fee will be applied). Note: No personal checks will be accepted. The cashiers check, money order, or a copy of the receipt verifying the confirmation number for the online payment must accompany your application form.
  4. You must mail your request to the Alabama Department of Education, address provided under 'Office Locations & Contacts' section. Note: Application forms are not accepted by fax or e-mail.
