ProcedureEdit

Sending request via mail

  1. Download Application form online. You may not request two different types of certificates using the same form.
  2. Prepare your payment, which may be in the form of a check or money order payable to "State Board of Health".
  3. Send the completed form along with your payment to:

Obtaining certificate in person

  1. Go to any county health department in the State of Alabama to obtain a certified copy of an Alabama death certificate. Most death certificates can be issued while you wait.
  2. Complete a request form and pay all necessary fees.

EligibilityEdit

Alabama law states that death certificates are confidential records with restricted access for 25 years from the date of death. Death certificates more than 25 years old may be obtained by anyone upon payment of the proper fee.

Death certificates less than 25 years old may be obtained by the following persons:

  • Mother or father of person named on certificate
  • Husband or wife of person named on certificate
  • Son or daughter of person named on certificate
  • Grandchildren of person named on certificate
  • Sister or brother of person named on certificate
  • Legal representative of family or estate
  • Informant listed on death certificate as providing information



FeesEdit

  • Fee to search for a death certificate: $15.00 (includes one certified copy of the death record or a "Certificate of Failure to Find")
  • Additional copies: $6.00 each
  • To expedite a request: $15.00 additional fee


Note: Your check or money order must be payable to "State Board of Health". Please do not send cash. Fees are non refundable.

Need for the DocumentEdit

A Death Certificate is a document issued by a registrar of vital statistics that declares the date, location and cause of a person's death.

A copy of the death certificate is often necessary when you apply for administrative procedures, such as execution of the will or making funeral arrangements.

The Alabama Center for Health Statistics began filing death certificates in 1908 for persons who died in Alabama.
