Apply In-Person

  1. To apply for a Death Certificate, you must first register the Death.
  2. The death has to be registered with the concerned local authorities within 21 days of its occurrence, by filling up the form prescribed by the registering officer. As a first step a death should be notified to the respective authorities at the earliest it occurs.

  4. A death can be reported and registered
    1. by the head of the family, in case it occurs in a house.
    2. by the medical in-charge if it occurs in a hospital.
    3. by the jail in-charge if it occurs in a jail.
    4. by the headman of the village or the in-charge of the local police station in case the body is found deserted in that area.
  5. Once the information is registered / reaches the Revenue department, the concerned citizen has to approach the authority and apply for the certificate on any working day.
  6. Applicant shall use the respective form as per the link to apply, If he wish to register on his own: Form Link
  7. Once the completed form along with documents were submitted, Authorities will accept the form verify the details to process and issue the certificate.
  8. The information of any Death is given to the related wards on prescribed form within 21 days and then certificate is issued immediately free of cost by the person employed at Registration ward.
  9. After 21 days till 30 days, the information has to be given as above on prescribed form to the related ward but now Rs 2/- is charged as late fee.
  10. After 30 days and till 1 year, a permission letter from Additional Health officer is to be presented for registration of death and a late fees of Rs 5/- is to be paid. This information on prescribed form is submitted to the related ward according to the birthplace.
  11. After 1 year delay, the order is to be presented from deputy DM and a late fee of Rs10/- is payable. Now the registration of death is done at Head office, Health department. Rs. 5/- is paid as the fees to get death certificate.
OR
  1. If you report orally, the registrar will enter the information in the appropriate reporting form and get your signature or thumb impression. You may have to furnish receipt from crematorium or burial ground to get the certificate.
Note: In case the death has not been registered within the specified time of its occurrence, the certificate is issued after due police verification ordered by the revenue authorities.


Apply On-line

  1. Please go to: online portal
  2. In the given page please select “Citizen Login (e-partner)” from the options provided on top of the page to reach next screen.
  3. Please login using the prompted details. If new user, then click the option “New User Registration?” and follow the screen prompt to register.
  4. Once login, user gets a page to view the details for the login. Here applicant can use the respective option to change the details, if not correct. Otherwise use the appropriate tab to get the next page.
  5. User shall select the respective choice for certificate from the list of menu option as provided next to Welcome.
  6. Choose the respective menu option and select Death certificate from drop down menu to get an option. Click on the option to get next page.
  7. In this page, user will get a form and should fill / select details as prompted followed by uploading documents (Aadhaar must) and hit applicable tab to reach next page for making online payment.
  8. In the Payment gateway page use respective mode to pay by filling required details.
  9. Once the online payment is over, user gets acknowledgement in Mobile as well as in system. Keep it safe for future reference.
  10. From this point onwards the process will be same as how they processed in apply in person option to issue the certificate.
  11. Applicant can download the certificate once the intimation on mobile reaches him for approval.


Print Certificate

  1. Please go to: online portal
  2. In the given page please select “Citizen Login (e-partner)” from the options provided on top of the page to reach next screen.
  3. Please login using the prompted details. Once login, User shall select the respective choice for printing certificate from the list of menu option as provided next to Welcome.
  4. Please follow the screen prompt to get the printout of the certificate by using the application number generated during registering the details.

  • Application form for Death Certificate : An affidavit specifying the date and time of death or death slip by doctor
  • Receipt from crematorium or burial ground.
  • If there is no record for birth / death from hospital, then the village head / regional councilor / ma. MP / MBS Certificate of signature and stamp with any one of the doctor should be submitted.

Need for the DocumentEdit

  • As per the Registration of Births and Deaths Act 1969, it is mandatory to register every death with the concerned State Government within 21 days of its occurrence. If not registered within 21 days, permission has to be obtained from the Registrar/Area Magistrate by paying prescribed fee for late registration and can get death certificate.
  • A Death Certificate is a document issued by the Government to the nearest relatives of the deceased, stating the date, fact and cause of death.
  • It is essential to register death to prove the time and date of death, to establish the fact of death for relieving the individual from social, legal and official obligations
  • To enable settlement of property inheritance, the family to collect insurance and other benefits.
  • Death certificate is an important official document issued for the purpose of settlement of all the belongings and benefits of the deceased individual to the relatives
