ProcedureEdit

Apply In-Person:

  1. The department of health and family welfare will maintain the list of debarred / blacklisted / disqualified pharmaceutical firms and products. In order to get such information, one shall approach the concerned health and family welfare (Department of Medical Health and Family Welfare) office for their respective area as appropriate.

  3. This list will be updated, when the concerned department authority issues debarment notice about a firm or product. The list will also be displayed in the respective state department portal.
  4. The applicant shall use the contact information from the above link to obtain details on how to apply for this procedure.
  5. If the application is accepted in person, then please visit the respective office and obtain the application form to apply, or write on a plain A4 sheet as advised.
  6. Please complete the application form. Attach the required documents which are mentioned in the “Required Documents” section of this page.
  7. The applicant shall pay the charges or fees as per the authority’s advice.
  8. Please complete the application form. Attach the required documents as per the “Required Documents” section of this page and submit to the concerned authority and follow the authority’s guidelines.
  9. The authorities will process the request. The details will be validated against the records accordingly. In case the verification requires additional validation with concerned ministry / department, the authorities will process the application.
  10. Once the processing is completed, the information about debarred/blacklisted/disqualified firms and products will be provided as per the government norms.
  11. The applicant has to pay the fees as per the authority’s request.
  12. The whole process shall be completed as per the timeline provided by the department.

EligibilityEdit

  • Applicant should be a citizen of India.
  • Those who are 18 years and above are eligible to apply.
  • As this information will be useful for the health care department person, hospitality department and pharmaceutical industry, the department authorities will be eligible for this procedure.

  • Fee information and other charges can be obtained directly from the department office while applying.



ValidityEdit

  • The obtained information will be valid till the period for which the firm or product is debarred/blacklisted/disqualified.
  • The firm / product may be debarred for a period of 6 months/3 years or as per the period mentioned by the department.

  • The department may debar/blacklist/disqualify a firm if the organization was convicted under prevention of corruption or under the Indian penal code for the time being or causing a threat to the public health.
  • Incomplete application will be rejected.
  • Avoid spelling mistakes while filling the application form.
