Apply In-Person

  1. Medical certificate should be obtained by approaching a Doctor near your area. So, applicant should approach the doctor and request for a medical certificate.
  2. Apply using the format of application as applicable
  3. Fill in the application form and attach a passport size photograph.
  4. Doctor (Registered Medical Practitioner) will issue Medical Certificate. In some cases…
  5. Go to a doctor to examine
  6. Based on the requirement, Doctor prescribes tests.
  7. Applicant should take the prescribed tests
  8. Doctor will look in to the test results
  9. He will issue the medical certificate.


Apply Online - Uttar Pradesh Apply for Medical Certificate Online with Images

  1. Go to the portal : link and select “online services” option from the menu and hit “Fitness Certificate” sub option from the drop down menu
  2. In the given screen user should enter register mobile number and other details as prompted to login using login tab. If in case of new user, use “For new registration” to get the prompts to register as new user.
  3. Once login, user will get the dashboard for online process.
  4. Please select “issuance of fitness certificate” from the provided set of tabs to get the screen with information. Please click the applicable option and hit proceed to reach fitness certificate dashboard page.
  5. Other tabs are for illness and status option. Status option will be useful in getting the status using the application number
  6. In the page, select general fitness certificate tab to get online application form. Select the box for I agree…. Statement and hit “proceed” to get next page
  7. Here, user should enter/select prompted details, upload the prompted documents and finally hit “preview” option to get the preview of the request.
  8. If the preview looks ok, hit submit to reach next page for acknowledgement with application number. Keep it intact for future reference.

NOTE: Some corporates have their own format for this certificate. In such case, one has to get the format and get certified with the medical practitioner

  • As a policy MNC’s will ask the candidates to submit a medical certificate
  • Few Govt. service requires medical certificate to ascertain the fitness

  • Minimum of Rs.200 will be charged ( This fees changes from Doctor to Doctor)
  • Medical certificate which is meant for overseas job requires certain level of examination as advised by the recruiter from overseas and fees for such certificate will be decided by them
