Apply In-Person
- To Correct Birth Certificate, the applicant has to approach the Department of Health and Family Welfare – link
- An application form will be provided to the applicant by the concerned officer. or it can be downloaded from the following link – Application form
- The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
- Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
- The applicant has to attach the photocopies of the required documents along with the application form while submitting.
- Submit all the documents with the completed form to the concerned officer.
- The applicant has to pay the prescribed fee as requested by the authority.
- After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
- Then the application will be processed further.
- Once the process gets completed, the applicant gets a call from the office regarding the application.
- This entire process can be completed within 7 - 14 days or as per the timeline advised by the respective department authority.

- Copy of Birth Certificate (self attested)
- Copy of School certificate of child/Father/Mother in case of correction in their name (self attested)
- Self Declaration (Describe the reason of correction) under state rule 11
- Witnesses of two credible persons like Sarpanch/ Panch / Municipal Councilor (MC) / Member of the Legislative Assembly (MLA) / Member of Parliament ( MP) or Gazetted Officer under state rule 11.
- Copy of Report of hospital / Nursing home in case of institutional delivery(self attested)
- Voter Card for proof of residence and relationship / Copy of Bank Passbook / Copy of Passport/ Copy of Driving License / Aadhaar Card (self attested)
- Copy of Birth / School Certificate of other children (If any) (self attested)
