Procedure

Apply In-Person:

1. Applicant shall approach their respective civic agencies (municipal/corporation/local body) of their respective area from whom the license for pet dogs was obtained.
2. Go to the respective office. The contact details are given below in the “Office Location”.
3. Meet the respective authority at the office. Applicant shall get the advice from the authority to be followed in order to register the change of address.
4. Collect the applicable application form.
5. Submit the completed application form along with required documents, as mentioned in the “Required Documents” section for showing proof of change in residential address.
6. Authorities will check the submitted application and announce the fees / charges. Please pay and get the receipt. Follow up with the authorities for notification regarding the status of the application.
   

Apply Online : - Change Address of Pet Dog Registration Online with images

1. Go to online registration portal : Apply Online
2. In the given page please click “PET REGISTRATION” option under Citizen Services session
3. User will reach a new page. Please click “Change Address” option under “Pet Registration”.
4. Search for your pet with the existing registration number.
5. Instruction screen will be displayed. Go through the details and click the close icon to close the screen.
6. Applicant will reach online application page. Please complete the prompts as appropriate. Upload photo of applicant and pet. Complete the payment section as applicable.
7. Once all the details are filled please click the check box for “I agree….” option to get the “Receive OTP” button. Please click “Receive OTP” button.
8. Applicant will receive OTP in the registered mobile number.
9. Enter the received OTP at the prompted space and click applicable option and follow the onscreen information to complete the online application.
10. Please save the acknowledgement for future reference.
11. This application will be processed further.
12. Applicant shall follow the department notification to get the change in address.
