Maharashtra - Obtain Still Birth Report (Certificate)

ProcedureEdit

Apply In Person:

  1. Get a Still birth Certificate Registration Form (a.k.a CRS Form 3) from the registrar's office of your municipal authority.
  2. The address of the office is provided in the “Office Locations & Contacts’’ section of this page. Or click on the following to view contact details Link
  3. If the child was born in a hospital, the form is provided by the Medical Officer In-charge.
  4. Download still birth report form Link
  5. For a detailed list of info necessary to fill the form, check the “Required Information” section of this page.
  6. Fill in the form within 21 days of death of the foetus.
  7. If still birth is not registered within 21 days of its occurrence, the still birth certificate is issued only after police verification.
  8. Once the verification of the records (date, time, place of birth, parent's ID proof, etc.) is done by the registrar, the Still birth certificate is issued to the applicant.
  9. Just to be sure, do a follow up with municipal authority, 7 days after applying for a Still birth certificate, to obtain the certificate.
  10. You need to provide a self-addressed envelope at the municipality office so that the Still birth certificate is posted to the respective address within 7-14 working days.
  11. The registration fee for a Still birth certificate is 20 INR with a late fee added if you are applying after 21 days of the foetus’s death.


Note:

  • In the case of multiple births, fill in a separate form for each child and write 'Twin birth' or 'Triple birth' etc., as the case may be, in the remarks column.


Apply Online:

  1. To obtain Still birth certificate, visit the website Link
  2. Look to the right, where you will find a Sign-Up button.
  3. To register, click on the sign up for 'General Public.'
  4. The Signup box will appear as a pop-up.
  5. Fill in all your valid details in the relevant text boxes, like username, place of birth and so on.
  6. If the field of the Registration Unit which shows the username is marked as active, it means that your area is valid for online registration.
  7. Enter the verification code and click on the Register tab.
  8. After registration, a ‘Thank You’ message will pop up along with a prompt to check your email ID to confirm the registration.
  9. Check your email inbox.
  10. It will ask you to set up a new password for login.
  11. Set it up and sign in once again.
  12. A form will pop up where you need to fill in the name of your child, his/her parents, and the location.
  13. For a detailed list of info necessary to fill the form, check the “Required Information” section of this page.
  14. Fill it and submit within 24 hours.
  15. Download a soft copy on your computer and also take a print out.
  16. Visit the office of the Registrar of your region
  17. The address is provided in the “Office Locations & Contacts’’ section of this page.
  18. Get the form attested either by him or the Sub-Registrar.
  19. Post that, the office will verify details such as place and time of birth, parents name, gender, address, nursing home/hospital etc.
  20. If the verification is all in place, you will get the Still birth certificate sent to your address in 7-15 days.
  21. Just to be sure, follow up with the office after 7 days.


Note:

EligibilityEdit

Any Indian parent of a stillborn foetus that reached at least twenty weeks gestation and died before birth can obtain a Still birth Certificate from the concerned registrar's office.


FeesEdit

The registration fee for a Still birth certificate is 20 INR with a late fee added if you are applying after 21 days of the event.

InstructionsEdit

  • A stillbirth (also called intrauterine foetal demise) is most often defined as pregnancy loss that occurs after the 20th week of pregnancy.
  • If your baby was born after 24 weeks, you will, by law, be required to register your baby’s stillbirth.

Need for the DocumentEdit

The registration still birth is compulsory under the provisions of Registration of Births and Deaths (RBD) Act, 1969, in all parts of the Country.


Information which might helpEdit
