Procedure

Apply In-Person:

1. To Apply for Widow certificate in Madhya Pradesh, the applicant has to approach Social Justice and Disabled Welfare department – link
2. An application form will be provided to the applicant by the concerned officer.
3. Applicant has to enter all information in the application form.
4. Applicant must make sure to carry all necessary documents that are mentioned under “Required Documents” section of this page. (Both photocopies and originals)
5. Applicant should attach photocopies of documents (whichever is necessary) along with the application form while submitting it at the office.
6. Fee for this procedure has to be paid as per authority’s request.
7. If all above mentioned procedures were followed properly by the applicant, the transport permit license will be provided within 30 days.
8. If there is no information provided regarding certificate by concerned authorities, applicant can visit the office to know status of their application.
9. If an application is rejected, the certificate fee shall be non-refunded.
10. The Processing time for `this procedure takes 4-6 weeks.

Instructions

• An applicant can apply for the Widow Certificate provided she is having a unique CAN number.
• If the applicant doesn't have a unique CAN Number, she must register for CAN to access the application form for Widow Certificate.
• User must have valid username and password.
• Make relevant and correct entries only.
• Recheck the entries before clicking on Submit button.
• Mandatory fields cannot be left blank.
• Mandatory/Optional documents should be attached as per requirement.
• Do not make false entries.
