ProcedureEdit

A Death has to be Registered within 21 days of occurrence.

  1. To Obtain a Death Certificate, applicant should Register the death in the Birth and Death Registrar office or local municipal office which is located in their District.
  2. A death can be reported and registered
  3. by the head of the family, in case it occurs in a house.
  4. by the medical in-charge if it occurs in a hospital.
  5. by the jail in-charge if it occurs in a jail.
  6. by the headman of the village or the in-charge of the local police station in case the body is found deserted in that area.
  7. by the person in charge of the vehicle if it occurs in moving vehicle
  8. by the person in charge of the Dharamshala if it occurs in Dharamshala
  9. Application form should be obtained from the concerned office or can be downloaded from the following Link:Link
  10. Please make sure you have all the documents listed in the “Required Documents” section of this page.
  11. Fill the application and attached the required documents in proper
  12. Submit the application form to concerned authority for processing
  13. Authority will verify the all details ,then they will issue the Death Certificate.



Late Registration

  1. If the death is registered 30 days after it occurs, a written permission from the registrar is required and a late payment is required.
  2. If registering death after one year of its occurrence, requires an order from the Executive Magistrate and the late fee has to be paid.

  3. Approach the staff of this centres to obtain Death Certificate after register a death
  4. Complete the downloaded application form and upload all required documents with the help of staffs.
  5. After successful submission you will get notification to collect the certificate or you can get printout of this certificate through CSC Centers

  3. Through this portal you may report death within 21 days of the occurrence. If the death crossed 21days of occurrence, you must approach the Registrar of your area.
  4. you must sign up to get your user id and password which is used to Login
  5. At the time of sign-up, users will be able to select where application is being used for registration of death
  6. Applicant (you) need to fill the complete death reporting form to the best oftheir knowledge and belief.
  7. After successfully submitting the application, users will receive oneConfirmation mail on provided email id
  8. The users will be informed about the status of the application throughe-mail immediately after receipt of the application by the concernedregistrar
  9. Details of the registered event can be informed by email or can be checked from the home page of the portal at any time by quoting the application reference number.

  • The document which contains the time and place of death.
  • Post-mortem report if death by crime or by accident.
  • For delayed registration, requires an affidavit from the judicial magistrate indicating the cause, date and place of death.

EligibilityEdit

  • Parents and Relatives of deceased can apply
  • Any citizen or any foreigners legal heir, the death of whose relative takes place in the State can apply



FeesEdit

  • There is no fee required for registrations that are made within 21 days.
  • If registering a birth after 21 days, then the applicant has to pay a fee of Rs. 1.
  • If registering a birth after 30 days, then the applicant has to pay a fee of Rs. 5.
  • If registering a birth after one year, then the applicant has to pay a fee of Rs. 10.
