If your permit is valid and it’s stolen or lost, then you would have to file a FIR with the local police station and the report is the vital document to apply for the duplicate permit
If it is torn / stolen / mutilated then then remains can be attached for applying

Apply in Person

To apply for a Duplicate Private Service Vehicle Permit in Karnataka the applicant has to approach the RTO office to which your area belongs to. Following link can provide you information related to the list of RTO offices: Contacts Link
An application form will be provided to the applicant by the concerned officer.
The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
Make sure that the applicant has to attach all the necessary documents that are mentioned in the “Required Documents” section of this page.
The applicant has to attach the photocopies of the required documents along with the application form while submitting.
Submit all the documents with the completed form to the concerned officer.
The applicant has to pay the prescribed fee as requested by the authority.
After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
An acknowledgment receipt will be provided. Please save it for future reference, it contains the application summary and contact details of the person to be contacted.
Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
The processing time for this procedure takes up to 10 - 15 days.

Private Service Vehicle Permit is valid for 5 years and issued for concerned region only.
Usually the renewal of permit can be obtained with Vehicle fitness certificate and FC as per authorities of concerned RTO.

Need for the DocumentEdit

Private Service Vehicle Permits are issued to institutions that operate the vehicle for their own use.
Whenever such permit is lost or damaged or missing, then the Duplicate for the permit has to be applied.
An application for a permit to use a motor vehicle as a private service vehicle shall contain the following particulars, namely:
Type and seating capacity of the vehicle.
The area or the route or routes to which the application relates.
The manner in which it is claimed that the purpose of carrying persons.
