Procedure

Apply In-Person:

1. To obtain Inhumation or burial license, please visit the Municipal Corporation/ Town Municipality/ Village Panchayat Office of your respective area.
2. Visit the Office and get the application form to obtain inhumation or burial license from the relevant department or you can write a requisition letter on plain A4 sheet.
3. Please fill the application with essential information and details and it should be attested with the applicant’s signature.
4. Make sure that you have all the documents that are listed in “Required Documents” section of this page. Attach all the important documents mentioned.
5. Then, it should be submitted to the relevant authority. Also pay the required fees for the application process at the office as directed by the officials. Please collect the acknowledgement slip for paid fee. Keep it safe for future reference.
6. And your application and attached documents will be passed for verification to check if there is any discrepancy.
7. After all the processes are completed, concerned officials will approve the request and proceed with issuance of Inhumation or burial license through any applicable means.

Note: Person requesting the death certificate is required to provide the evidence of relationship with the deceased and complete address with nationality
