India - Obtain Death certificate for Animals

ProcedureEdit

Apply In-Person:

  1. To obtain a death certificate for the animal, the applicant must approach the Veterinary Hospital of their respective area.
  2. Applicant must register the death of the animal by filling the prescribed form.
  3. Collect the applicable application form from the office.
  4. Fill the application form with the required information.
  5. Attach the required documents as per details given in the “Required Documents” section of this page.
  6. Submit the application form at the Veterinary Hospital office and make the required payment.
  7. Death Certificate is then issued after proper verification.

  • Aadhar card of the owner
  • Passport size photos of the owner
  • Residential proof of the owner (acceptable residential proof are listed below)
  • Water Bill
  • Telephone (landline or postpaid mobile bill)
  • Electricity bill
  • Income Tax Assessment Order
  • Election Commission Photo ID card
  • Proof of Gas Connection
  • Certificate from Employer of reputed companies on letterhead
  • Spouse’s passport copy
  • Parent’s passport copy, in case of minors(First and last page)
  • Aadhar card
  • Rent Agreement
  • Photo Passbook of running Bank Account
