Apply In Person

1. Here are the procedures on how you can replace your lost or stolen PhilHealth card. For this procedure you will need to personally go to any Philhealth branch near you and submit an application. You can use the link provided under 'Office Location & Contacts' section to view the addresses of different PhilHealth Regional Offices, Local Health Insurance Offices, Business Centers and PhilHealth Express Branches.
2. You will be required by PhilHealth to present an Affidavit of Loss. To help you draft one, you may go to a notary public.
3. Inform a PhilHealth officer that you would like to replace your PhilHealth card. The office will give you a request form to fill out.
4. Submit the completed form together with the Affidavit of Loss and two valid IDs.
5. Pay the replacement fee at the cashier.
6. After you have submitted your request, you can stay and wait for the release of your ID. Your waiting time would depend on the number of persons currently being serviced by the office.
