This procedure explains various ways to Renew a Passport in Zamboanga City.

Apply OnlineEdit

  1. Here's how you can renew your Philippine Passport online. First, you will need to book an appointment using DFA’s Passport Appointment System. Click this - LINK.
  2. To begin, click the “Schedule an Appointment” button.
  3. Read the Terms and Conditions page. After reading, proceed with the booking process by clicking the “START INDIVIDUAL APPOINTMENT” button. NOTE: Option to book an appointment for a group is also available.
  4. The appointment page will open. Here you will be asked to indicate where you would like to process your application. Make use of the drop down menus to select your region and country. For applicants in the Philippines, you don’t need to change anything as the default setting is already set locally (DFA sites within the Philippines).
  5. Next, indicate the specific DFA site where you would like to process your application. Use the drop down menu and scroll through different DFA offices.
  6. After selecting a DFA site, click the “NEXT” button.
  7. Next, you will have to set a date and time for your appointment. If you notice, the calendar is set to show the current month and year. In case you would like to apply for a different month or a different year just click on the month and the year text.
  8. Once you have set a date and time for your appointment, tick the captcha box and then click the “NEXT” button
  9. Complete the Personal Information Form.
  10. After you have finished filling out the Personal Information Form click the “NEXT” button. Next, you will need to fill out the Family Information Form. Again, once you're done, click the “NEXT” button.
  11. On the next page, you will be asked what type of application you are applying for. Since you are applying to renew your passport, select application type as “Renewal” from the drop down menu.
  12. Complete the rest of the form. You will need to provide your old passport number, the date it was issued and the issuing authority. You will also need to state the basis of your Philippine citizenship and designate an emergency contact person. Click "Next" after.
  13. Now, enter your contact information. Click "Next" after.
  14. You will be taken to the Information Review page. Go over your application once more before finally submitting it for processing. Make sure that you double check everything (specially your name) because the DFA will exactly use the data that you have encoded when they process and print your passport.
  15. When you finish reviewing your application, tick the captcha and consent boxes, and then, click the “CONFIRM” button to finally submit your application to the DFA.
  16. Now, you are done with the first part of the application process which is setting up an appointment with the DFA. DFA will email you a copy of your completed form. Print and bring that form with you on the day of your appointment along with all the other required documents.

Apply In PersonEdit

  1. After you have set up your appointment with DFA online, to complete the renewal process you need to go to your selected DFA office or branch and submit your request in person.
  2. On the day of your appointment, print and bring with you the form which DFA emailed to you. If possible arrive at least 30mins before your scheduled appointment.
  3. Get a queue number at the Information Counter and sit on the waiting section of the Processing Area. Wait for your turn.
  4. Once called, present all documents for review by the officer.
  5. Pay the applicable passport processing fee.
  6. After payment, proceed to the Encoding Section for data capturing. This is the last part of the process. You can personally pick up your new passport once it’s ready or, you can also have it delivered to your home (optional).
  7. When you collect your new passport don't forget to bring the Official Receipt.

ADULTS:

  1. Accomplished Application Form
  2. Current ePassport with photocopy of data page
  3. Original PSA authenticated documents that will support the change of name.
    • Marriage Contract. Note: If the applicant opts to retain her maiden name, a Marriage Contract is not required.
    • Annotated Birth Certificate
    • Annotated Marriage Contract to show annulment/divorce/court ordered instruction. Note: If the applicant opts to retain her maiden name, a Marriage Contract is not required.
    • Death Certificate of Spouse


MINORS:

  1. Accomplished Application Form
  2. Current ePassport with photocopy of data page
  3. Marriage Certificate is required if only one parent is accompanying the child
    • If parents are unmarried, an Affidavit of Support and/or Consent executed by the Mother will be required if she is not accompanying the child
    • If Affidavit of Support and/or Consent is executed abroad, it must be authenticated by the Philippine Embassy or Consulate
  4. Original and photocopy of proof of filiation and/or guardianship
  5. Passport or Valid Government issued ID of either parent with one (1) photocopy
  6. School ID (if applicable)


List of Acceptable IDs for Philippine Passport Application

  • Social Security System (SSS) Card
  • Government Service Insurance System (GSIS) Card
  • Unified Multi-Purpose Identification (UMID) Card
  • Land Transportation Office (LTO) Driver’s License. Student Permit may be accepted if in card format.
  • Professional Regulatory Commission (PRC) ID
  • Overseas Workers Welfare Administration (OWWA) E-Card
  • Commission on Elections (COMELEC) Voter's ID or Voter's Certificate issued from COMELEC main office in Intramuros, Manila (Certificates issued in COMELEC's regional offices may require further verification. Applicants are requested to present NBI or police clearance along with the Voter's Certificate).
  • Philippine National Police (PNP) Permit to Carry Firearms Outside Residence
  • Senior Citizen ID
  • Airman License (issued August 2016 onwards)
  • Philippine Postal ID (issued November 2016 onwards)
  • Seafarer's Record Book (SRB)(*must be issued Feb 2020 onwards) issued by the Maritime Industry Authority (MARINA)
  • Valid or Latest Passport (For Renewal of Passport)
  • For minor applicants, School ID OR Certificate of Enrolment with photo of minor and dry seal of school may be presented
  • Notes:
    • For applicants based abroad, you may use your host government issued IDs showing your Philippine citizenship. (Example: Residence Card)
    • All IDs presented should be readable, untampered and contains consistent information with the documents presented upon application.
    • Original and photocopy of IDs and supporting documents are required to be presented upon application.

It will take about twelve (12) working days for your application to be processed. The expedited process takes 6 (within Metro Manila) to 7 working days (outside Metro Manila).

  • The renewal process is in two parts. First, you need to book an appointment with DFA online. And second, to complete the procedure you will need to go to the DFA office in person.
