This procedure explains various ways to Register a Co-operative in Zamboanga City.

Apply In PersonEdit

  1. Here are the procedures on how you can register a co-operative in person. Application are processed by Cooperative Development Authority (CDA). You can submit your application to any CDA regional offices (address and contact numbers are provided under "Office Locations & Contacts" section).
  2. Begin your application by completing the necessary forms. Links are provided under "Documents to Use" section which you can click to download the forms. For the Treasurer's Affidavit, Articles of Cooperation and By-Laws, make sure that you download and complete the form that corresponds to the type of cooperative you are about to register. There is also a guideline which you can download to help you in filling out the forms.
  3. Submit all accomplished registration forms and other documents to CDA Regional Office's clerk or receiving staff.
  4. After receiving your application, CDA will schedule and conduct an on-site validation and verification of the co-op within 10 days.
  5. After the on-site visit, CDA will evaluate all submitted registration documents.
  6. The Senior Cooperative Development Specialist (CDS) and the Accountant will prepare and issue the Statement of Account (SOA).
  7. Pay the corresponding registration fee at the CDA Cashier.
  8. Submit the duplicate copy of the Order of Payment and present the Official receipt to the Senior CDS for the issuance of the Certificate of Registration.

Apply OnlineEdit

  1. Here's how you can register a co-operative online using Cooperative Development Authority's Cooperative Registration Information System. But before you can use the online facility, you must first create an account (in case you don't have one.)
  2. To create an account, please follow these steps:
    • Click the this LINK.
    • When the Cooperative Registration Information System Log-in page opens, click on “Create an Account” and enter the information required: Name, Birth Date, Mobile Number. You will also need to provide an email address which you will use to log-in to your account after registration.
    • Input your home Address and password
    • You will also need to provide a valid ID and ID no.
    • After completing the form click the "Submit" button.
    • Check your email to verify your account.
    • Once verified, you may now login to your account and begin your application.
  3. First step in registration is reserving your cooperative's name.
    • To reserve the name, click the "New Registration" button.
    • Select the following required information: Category and type of co-op, major industry classification and subclass (if applicable).
    • Enter the “Proposed Name” (if proposed name already exists in the database, the system will prompt user *This cooperative name is not available. Important: Do not include the word “Cooperative”; “Federation”; “Union” and even the Type of Cooperative. CDA's system will automatically display the type of your cooperative and the word “Cooperative” after submission of form. (e.g. Agriculture Cooperative; Labor Services Cooperative; Credit Cooperative). It is also better if you do not input the tagalog version of “Cooperative Type” and “Cooperative”(e.g. Samahan ng mga Magngingisda – as it will looks like this “Samahan ng mga Mangingisda Fisherman Cooperative)
  4. Complete the rest of the form and once you are done click on "Submit".
  5. The Cooperative Information Overview page will be opened. You can see on this page all the steps that you need to completed (13 in total). You will see which steps you have completed and the rest of the steps you will have to go through. Remember, you will need to complete the step with a "Pending" status before you can move on to the next. Like for example, since you have completed the first step which is "Name Reservation and Basic Information" step, you can now move on to the next step which is completing the By-Laws form.
  6. As you complete the By-Laws Form keep in mind that whatever information you enter will appear in the By Laws document so fill out the form correctly. When you are done, click the "Submit" button (your application will not be submitted if there are any fields that failed to comply on the requirements. System will show which field(s) should be amended, changed or requiring information.) When the completed By-Laws Form is accepted the next step / form will be open.
  7. The next form you will need to complete is capitalization. Like what you did in step 2, you will need to complete the form, provide all the needed information and submit it once you are done. You will be doing the same process for each steps until step 9.
  8. Step 10, here you will be able to view your co-op's By-Laws, Article of Cooperation, Economic Survey, Treasurer Affidavit. Review carefully each document and make sure that all the information are correct and accurate. In viewing the Article of Cooperation, you will encounter this message, “If total number of pages in Acknowledgment didn’t appear, please refresh the page”. Before you download and print the Articles of Cooperation, make sure that you see "Total No. of Pages in Acknowledgement appeared" prompt. To complete step 10, upload required document for Bond of Accountable Officers and Pre-Registration PRS Certificate and other required documents according to Type of Cooperative.
  9. Next, Step 11, finalize and submit your application. To proceed to the application evaluation step, click on “Submit” button. Once submission of your application is confirmed the evaluation process will begin.
  10. When the evaluation of your application is completed, you will receive an email stating, “Your application status is FOR PRINTING AND SUBMISSION”. The email also contains instructions on how you can complete the final steps.
  11. Submit all required documents listed on the email.
  12. After submission, wait for an e-mail notification of payment procedure.
  13. When you receive the email notification, look for the "Payment" button. When you click it the system will display the Order of Payment Form. Choose between:
    • Pay at CDA Treasurer
    • Pay Online (thru Landbank)
  14. Click on the button of your choice.
  15. If you have selected “Pay at CDA Cashier”, print out the Order of Payment and pay at the Cashier of CDA to get your Official Receipt and Certificate of Registration.

Basic Requirements:

  1. Completed Economic Survey
  2. Articles of Cooperation and the approved By-Laws:
    • All original
    • The Articles of Cooperation must be signed by all the cooperators on each and every page.
    • The By-Laws must be signed by at least majority of the members on the adoption page.
  3. Treasurer's Affidavit
  4. Surety Bonds of Accountable Officers
  5. Certificate of Pre-Registration Seminar (PRS)


Other specific requirements for each type of cooperative:

  1. Multi-purpose:
    • Detailed feasibility study
    • Undertaking to maintain separate books of accounts for each business activity
  2. Agrarian reform beneficiaries:
    • Mother CLOA in case of plantation based ARBs
    • Written verification from Department of Agrarian Reform (DAR) to the effect that the cooperative organization is needed and desired by the beneficiaries, economically viable, at least majority of the members are agrarian reform beneficiaries.
  3. Housing:
    • Copy of the Pre-feasibility Study of the housing projects undertaking certified as reviewed by National Housing Authority (NHA).
  4. Transport:
    • Certification of Cooperative Education and Transport Operation Seminar (CETOS) by Office of Transport Cooperative (OTC).
    • Favorable endorsement by OTC.
  5. Water service:
    • Proof of Land Ownership
  6. Professional Service Cooperative
    • Favorable endorsement from the governing Board of the respective profession.
    • Tax identification numbers of all cooperators.
    • Photocopy of valid individual Professional License of all members
  7. Labor service:
    • Cooperators/Member's profile.
    • Tax identification numbers of all cooperators.
  8. Health service:
    • Favorable endorsement / written certification from concerned health related professional regulatory board attesting to the fact that the concerned health related profession is not prohibited from forming a cooperative for the purpose of engaging in the subject undertaking.
    • Tax identification numbers of all cooperators.
    • Detailed Feasibility Study (expressly mentioning whether the undertaking is primary, secondary or tertiary level hospital, diagnostic center, spa, and wellness center, home for the aged, lying in, drop-off center, etc. and specifying the financial, technical including architectural, plans, etc.)
  9. Small scale mining:
    • Certification from the Mines Geo-Science Bureau Regional Office that the members are licensed miners if the area of business operation is within the People's Small-Scale Mining Area.
    • Tax identification numbers of all cooperators.

EligibilityEdit

  1. Cooperative must at least have fifteen (15) or more members who are Filipino citizens, of legal age, with common bond of interest and are actually residing or working in the intended area of operation.
  2. Has completed a Pre-registration Seminar (PRS)
  3. Capital Requirements for the Co-op:
    • At least 25% of the authorized share capital is subscribed by the member and at least 25% of the subscribed share capital was paid by the members prior to registration.
    • The paid up capital is not less than Php15,000 except for multipurpose cooperative which should have at least Php100,000 or as required by the feasibility study, whichever is higher.
    • In the case of Transport Cooperatives, capital requirements is based on Rule V Section 5 of IRR, RA9520

Name Reservation (Pre-registration):

  • 30 calendar days: Php100.00
  • 60 calendar days: Php200.00
  • 90 calendar days: Php300.00


Initial Registration: The initial registration fee is equivalent to 1/10 of 1% of the authorized share capital or the basic fee below whichever is higher.

  • Primary cooperatives: Regular lane - Php500.00 / Express Lane - Php1,000
  • Secondary cooperatives: Php2,000.00
  • Tertiary cooperatives: Php3,000.00
  • Laboratory cooperatives: No fee


Other fees:

  • Certification: Php100.00
  • Appeal Fee: Php300.00
