This procedure explains various ways to Obtain a Duplicate Seaman Book in Zamboanga Peninsula.

- Here are the procedures on how you can obtain a duplicate Seafarer’s (Seaman) Record Book (SRB) in the Philippines. The application process is in two parts. First, book an appointment online using MARINA’s SID/SRB Online Appointment System, here’s a link to the site: MARITIME INDUSTRY AUTHORITY SID/SRB Appointment System.
- To start the online process click the “SCHEDULE AN APPOINTMENT” button.
- A pop-up window will appear. It contains the site’s terms and privacy policies. Scroll through the page to read the rest of the document. On the bottom part of the document you will see the “I Agree to the Terms & Privacy Policy of this Website” button, click it to move on to the next step.
- Another pop-up will appear. This time you will be required to enter your email address on the box provided. After typing in your email, tick the captcha box and then click the “Submit” button.
- You will be taken to the appointment page. There you will be asked to set an appointment for your schedule. Just a few notes before you begin, take a look at the timer located on the upper right side of the page. You have 60mins to complete your application. The timer will tell you how much time you have left. Make sure to complete the online process before you run out of time.
- Now, let’s begin setting up your appointment. First, select the MARINA site where you would like to apply. Use the drop down menu to scroll through different MARINA offices. Click the site of your choice.
- After selecting a site, a calendar will appear. Use the calendar to pick a date for your visit. Simply click on the date of your choice on the calendar.
- Next, you will need to pick a time for your appointment. A pop up will appear showing time slots available for your chosen date. To select a time just click the button opposite the time of your choice.
- On the next page you will be asked to select the type of transaction you are applying for. First, select your seafarer category and type using the drop down menu. Next, tick the (SRB) Seafarer’s Record Book box. Then select whether you are requesting a duplicate copy for your lost or damaged SRB. Last, select your application type: regular or on board. Once you are done, click the “Next” button.
- Next, you will need to complete the application form. After filing it up click the “Next” button to move on to the next stage where you will be asked by the system to upload the required documents and pay for your application. Note: Uploading your documents is optional. Should you choose to skip this step make sure to bring the said documents on your appointment with MARINA.
- After completing your online transaction a confirmation will be sent to your email.
- On your appointment date, bring the printed application form with you, together with all the original copies of the documents you have submitted via the online appointment system (documents you have uploaded). You can collect your Seaman's book on the same day if you have successfully accomplished all needed qualifications and have paid all the necessary fee.
Note:
- Seafarers will no longer be issued a Seafarer’s Identification and Record Book (SIRB) starting November 2019. Instead, they must apply for the new Seafarer’s Record Book (SRB).
- A Seafarer’s Identity Document (SID), is different from SRB. It can be presented as a form of identification. You may or may not opt to apply for a SID, as long as you have secured a SRB which is a requirement for all seaman.
- You can apply for both documents online through Marina's SID/SRB Online Appointment System.

- Letter request from Company / Agency
- POEA Approved employment contract
- Clearance of no pending case/obligation or unpaid penalty from Legal Service
- Affidavit by the Seafarer and confirmation by the Master
- Company Affidavit of Undertaking (duly notarized)
- Two (2) Documentary Stamps
Seafarer’s (Seaman) Identity Document (SID):
- Valid SRB or SIRB
- Duly Notarized Affidavit (for Lost or Damaged)
- POEA Approved employment contract
- Crew list duly signed by the Master
- Clearance from Legal Service as to monetary obligation
- Affidavit by the Seafarer and confirmation by the Master
- Company Affidavit of Undertaking (duly notarized)

EligibilityEdit
A seaman or seafarer who previously owns a Seaman's Book (SRB and/or SID). You can apply for the procedure in case you have lost your documents or would like to replace a damaged one.

Seafarer’s Record Book (SRB):
- Re-issuance Regular: ₱ 1,000.00
- Re-issuance On-Board: ₱ 1,800.00
Seafarer’s Identity Document (SID):
- Re-issuance Regular: ₱ 600.00
- Re-issuance On-Board: ₱ 1,000.00
Payment Convenience Fee (Additional Fee on top of the Processing Fee)
- Credit/Debit Card: 5% + ₱ 6.00
- Cash: ₱ 50.00
- eMoney: ₱ 50.00
- Online Banking: ₱ 50.00
ValidityEdit
Seafarer’s Record Book (SRB) is now valid for ten years. In the event that the pages are full, you will have to renew it even if it hasn't expired yet.

Processing TimeEdit
Processing time from setting an appointment to the release of your Seafarer’s Record Book (SRB) will depend mostly on how early you can book an appointment with MARINA.
When choosing an appointment date, you might want to check out MARINA's Satellite offices in the province first since they usually have more vacant slots.
On the day of your appointment, you can collect your Seaman's book within 3 hours or so if you have successfully accomplished all needed qualifications and have paid the application fee.
