ProcedureEdit

Apply In-Person

  1. To replace a damaged National Identity Card in Zambia, the applicant will have to contact the department of National Registration Passport and Citizenship office. Please tap on the following link to view contact details link.
  2. Make sure to have all the documents that are listed under the “Required Documents” section of this page.
  3. Upon visiting, the applicant will be provided with an application form to be filled in the district office. Find the form on page 20 of this pdf file application form.
  4. Once understanding the guideline properly, the applicant has to fill in all the necessary information in the application form.
  5. Submit the application form with necessary attachments to the office.
  6. Pay the fee as given in the “Fees” section of this page.
  7. The application will be duplicated for local and central offices.
  8. Upon the verification of all required documents, the updated ID card will be issued
  9. Collect the National Identity Card from the office in 5 days or wait for it to arrive via post on the provided postal address in a stipulated period of time.

  • In any case where a national registration card is materially damaged, the person to whom it was issued shall, without undue delay, report destruction or damage to the nearest registrar.
