Zambia - Obtain License for Hospital (Health Facility)

ProcedureEdit

Apply In-Person:

  1. To apply for a hospital (health facility) license in Zambia, the applicant will have to contact the Health Professions Council office of Zambia, for the main office visit link.
  2. Make sure that you have all the documents that are listed under the “Required Documents” section of this page.
  3. The application form can be obtained from any nearest office of the Health Professions Council, find the form here form.
  4. Fill the application form provided to you after understanding the guidelines properly.
  5. The health inspectors will inspect everything according to the guidelines provided by the Health Professions Council.
  6. If the facility meets all the guidelines and the health inspector signs the application form, the applicant will most probably get the license.
  7. Licensing fees should be paid once the facility meets the requirements.
  8. Pay the fee as per given in the “Fees” section of this page.
  9. Upon the satisfaction of the authorities, a provisional certificate will be accessible.
  10. The applicant gets the license in a stipulated period of time.

Required DocumentsEdit

  • Photocopy of the registration certificate with the registrar of companies/ societies (e.g. Business Name Registration, Companies Certificate of Incorporation).
  • Photocopy of ZRA Tax registration certificate.
  • Photocopy of National Registration Card(s) or Passport(s) of Proprietor(s).
  • Photocopy of Business Permit from local authority.
  • Photocopy of Fire Certificate from local authority.
  • Copies of registration and practicing certificates for all health practitioners intending to work at the facility including nurses.
  • Copy of proof of ownership of premises or if premises are leased, copy of tenancy agreement Environmental Impact Assessment (Only applicable to newly built facilities).
  • Copy of proof of payment for application, inspection.
  • MOU or signed contract with companies disposing medical waste-Only applicable for facilities with no approved incinerator Letter from the supervising health practitioner.

EligibilityEdit

  • A person who holds a qualification from a training institution recognised by the Health Professions Council of Zambia.



FeesEdit

  • Application Fee(All facilities & services Clinic) for Zambian & Non Zambian: K 450.00.
  • License Fee: Depends on the type of facility.



ValidityEdit

  • One year and should be renewed for the following year must be renewed before 31st December of the preceding year.

  • In 30 days, the application will be granted or rejected. When granted the applicant gets the license in a stipulated period of time.

  • The Licence is not transferable to another health facility, person or persons.
  • The health facility should comply with all the requirements set for health facilities.
  • The Health Professions Council of Zambia should be informed in writing before the health facility is relocated to new premises.
  • The Health Professions Council of Zambia should be informed in writing before the facility changes its name.
  • The Health Professions Council of Zambia should be informed in writing when the ownership of the facility has changed.

  • Supervising Practitioners Details(Name, Nationality, NRC /Passport No., Profession, HPCZ, Registration No., Workplace Address, Residential Address, Phone No, Email Address).
  • Services intended to be provided at the health facility.
  • Names of all the staff intending to work at the facility.
