ProcedureEdit

Apply In-Person:

  1. To apply to make representation to a Parole Board in person, the applicant has to visit the office of the relevant Correctional Supervision and Parole Board.
  2. The contact and address details of the office of CSPB can be found in the following link: Link Click “Regions” tab to get regional offices’ contact and address details.
  3. Visit the relevant office of CSPB and obtain the specified format to inform the board about your wish to make representation to Parole board.
  4. Else click to download the Request to make a representation to a Parole Board form: Link
  5. Make sure that you have all the documents required for the process as mentioned by the board.
  6. Complete the necessary format and attach the documents with it. Now, submit it to the relevant authority at the office of CSPB.
  7. The submitted documents will be sent to the relevant officials of the board for examination and verification processes.
  8. Once all the necessary processes got over, the applicant will be informed about the further processes that should be done to complete the process.

  • Any person, who is a victim of a crime, may apply to make a representation to a parole board.
  • The complainant, who lodged the complaint on behalf of the victim, can also apply for making a representation.

InstructionsEdit

  • After lodging the request form, the board will inform you in writing, stating when and to whom you should make representations. The date and venue of the relevant parole board meeting will also be specified.
  • It is your responsibility to inform the commissioner or CSPB of your change of address (if any) before the representation.

Need for the DocumentEdit

As a complainant or victim of a crime, you may make representations in writing or attend the Parole Board sitting when the offender is considered for placement on parole or under correctional supervision, by properly obtaining permission from the Correctional Supervision and Parole board.
