Procedure

Apply In-Person:

1. To Apply FSSAI license for Manufacturing Potato Chips, you have to approach the Commissioner of Food Safety / Officer-In-Charge for Food Safety.

3. You can download the application Form-B from the following link:Form B.
4. Please make sure you are eligible to make the application by going through the eligibility criteria provided under the “Eligibility” section of this page.
5. First download the Form-B from the link and fill all the required information and you need to submit the form to the Commissioner of Food safety department. The address is mentioned in the office locations & contacts section below.
6. Please complete the application form and make sure you have all the documents that are listed under “Required Documents” section of this page.
7. Pay the appropriate fees and submit the form to the Officer-In-Charge in the Food Safety department.
8. Now the authorities will verify your application and validate the submitted documents.
9. Authorities will inspect the facilities that are related to the Manufacturing of Potato Chips and if they are satisfied with the standards followed the authorities will provide the franchise permission.
10. The applicant will be later requested to collect the FSSAI License for Manufacturing Potato Chips on a specified date.

Apply Online:

1. To Apply FSSAI License for Manufacturing Potato Chips, through online, you can use the FOSCOS website. Please click the following link : FOSCOS Website
2. Click the “Login-Businesses” tab and Under User sign-in Box enter your Application Ref. No./License No./Certificate No. and Click on the “Proceed” button to login.
3. If you are a new user, then Click on the "Do you want to apply for New License/Registration" and click on the "Apply for License/Registration" tab in the new page.
4. Next select the state from the dropdown list box and read the Note.
5. Next select the type of Business, Now Select the annual turnover of your business. After that Click on the “Proceed” tab.
6. On the next page, Click on the “Click here to apply State License for all Businesses”.
7. Next Form-B page will be displayed, fill all the required informations under premises details section and Click on the “Save & Next” tab.
8. On the Production selection page, fill all the details for DGFT Authentication and Click on the “Proceed with DGFT validation” to authenticate the details.
9. Next Click on the “Save” to save the authenticated details. After Successfully Proceeding with DGFT validation, Click on the “Save & Next”.
10. Next fill all the Communication details and Click on the “Save &Next”.
11. Under required documents section upload all the relevant documents and Click on the “Save & Next”.
12. Next pay the fees and submit the application.
13. After Successful Completing the payment, a receipt will be generated with a 17-digit reference number make a print of it and keep it safe for future reference.
14. You can track the application through the Home page.
15. This application will be processed by them and they will notify you through your e-mail or by SMS.

Apply through mobile app:

1. The applicant willing to apply for FSSAI License for Manufacturing Potato Chips must download the mobile app Food Safety Connect App.
2. The applicant can download the app from playstore using the following link. Link: link
3. Once downloaded, the applicant must click the button “Apply for license” in the main page.
4. Once selected, a new page opens. In the newly opened page, the applicant need to click the option “Apply for License / Registration”.
5. In the newly opened page, the applicant need to choose their state and click on the button “Manufacturer” and click “Proceed” at the bottom of the page.
6. Now a new page opens which shows the eligibility to apply for the license. The applicant need to read and check if they satisfy the eligibility conditions and click “click here to apply for central license for all businesses” button.
7. Now the applicant can close the application

• Identity Proof Documents such as Ration Card, Department Issued ID, PAN Card, Passport, Senior Citizen Card, Aadhaar Card, Driving License, Voter ID Card
• Blueprint/layout plan of the processing unit showing the dimensions in metres/square metres and operation-wise area allocation (mandatory for manufacturing and processing units only)
• Id proof and Address Proof.
• List of Directors with full address and contact details (mandatory for companies only)
• Name and List of Equipments and Machinery along with the number, installed capacity and horse power used (mandatory for manufacturing and processing units only)
• List of food category desired to be manufactured. (In case of manufacturers)
• Analysis report (Chemical & Bacteriological) of water to be used as ingredient in food from a recognized/ public health laboratory to confirm the potable (mandatory only for manufacturing and processing units only)
• Food Safety Management System plan or certificate (if any)
• Declaration Form 8. Upload Production unit photograph
• Photo I.D and address proof issued by Government authority of Proprietor/Partner/Director(s)/Authorised Signatory
• Proof of possession of premises. (Sale deed/ Rent agreement/ Electricity bill, etc.)
• Partnership Deed/Self Declaration for Proprietorship/Memorandum & Articles of Association towards the constitution of the firm
• Form IX: Nomination of Persons by a Company along with the Board Resolution
• Additional documents(If any)
• Form.
