Apply In-Person:
- To apply for lodging a complaint on any member of the South African Police Services and Municipal Police Services in person, the person who wants to lodge a complaint must visit any office of Independent Police Investigative Directorate of South Africa.

- Visit the link and find the contact and address details of the particular province that is applicable to you.
- Pay a visit to the IPID office and obtain the application form (Form 2) to file a complaint against any SAPS or MPS member. The application form can also be downloaded by clicking on the following link:Link
- Fill all the required details about the complaint like date and place of the incident, proper mention and description of the incident under the “Complaint details” part of the form.
- Also, provide details about the complainant (the person who lodges the complaint) like ID number, personal details, contact details, gender and Nationality under the “Complainant Details” part. Please provide the details appropriately.
- You will also be required to fill the details of the Victim like Passport number, gender and personal details under the “Victim Details” part.
- Then details like ID number, personal details and Duty Station details of the Service Members should be filled with proper care and legibility under the “Service Member’s Details” part.
- At last, personal details and contact information of the Witnesses to Incident are to be filled and the form should be attested by the Complainant’s Signature with date.
- Then, the completed Complaint form should be submitted at the IPID office along with the documents mentioned in “Required documents” section of this page, for further processes. After the necessary processes, disciplinary proceedings will take place.
Apply through Telephone
- To apply for lodging a complaint on any member of the South African Police Services and Municipal Police Services through phone call, the person who wants to lodge a complaint must contact any office of Independent Police Investigative Directorate of South Africa through telephone.

- Visit the link and find the contact details of the particular province that is applicable to you.
- Contact the IPID office and inform that you want to lodge a complaint against a member of the SAPS or MPS.
- You will have to provide personal and contact details of the complainant, victim as well as the witnesses of the incident.
- Please cooperate with the officer at your service as instructed and provide the required information asked by them.
- After giving complete details, your application to lodge a complaint will be submitted and forwarded to the respective department to get verified by the concerned authority.
- After the necessary processes, disciplinary proceedings will take place.

- To apply for lodging a complaint on any member of the South African Police Services and Municipal Police Services through letter, the person who wants to lodge a complaint must fill a Complaint Reporting form and send it to the office of Independent Police Investigative Directorate of South Africa through a letter by post.

EligibilityEdit
- Any person who wants to lodge a complaint against any misconduct or offence committed by any member of the SAPS or MPS can apply.
- The person who applies can be a victim, a witness or a representative.
- Any non-governmental organization or community-based organization can also apply to lodge a complaint.
- The person applying should possess a valid South African Identification Document. The victim should possess a Passport which will be required at the time of filling application form.

Need for the DocumentEdit
- The mandate of the IPID is to conduct independent and impartial investigations of specified criminality committed by members of the South African Police Service (SAPS) Municipal Police Services (MPS). And this mandate requires the complainant to lodge a complaint by submitting a Complaint Reporting form.
Information which might helpEdit
- The Directorate must take up the cases related to any deaths in the custody of Police and deaths caused by actions of the Police.
- Any complaint regarding discharge of official firearm by any police officer should be investigated by the Independent Police Investigative Directorate.
- It should also investigate any rape committed by a police officer either in on or off duty and rape of a person in police custody.
- Any complaint of torture or assault against a police officer in the execution of his or her duties, must be investigated by the Investigative Directorate.
