Procedure

Apply In Person

1. To Register a Birth, the applicant has to visit the Immigration & Checkpoints Authority contact link or the registration can be done in any designated birth registration centre located at:
   
   • Gleneagles Hospital link
   • KK Women’s and Children’s Hospital link
   • Mount Alvernia Hospital link
   • Mount Elizabeth Hospital link
   • Mount Elizabeth Novena Hospital link
   • National University Hospital link
   • Parkway East Hospital link
   • Raffles Hospital link
   • Singapore General Hospital link
   • Thomson Medical Centre. link
2. Get the application form to register a birth from any above given hospital or centres.
3. The applicant has to fill all the necessary details in the form.
4. Make sure that you have attached all the documents mentioned in the “Required Documents” section of this page.
5. Submit the completed form along with the documents to the respective officer.
6. Fee for this procedure is given in the “Fees” section of this procedure.
7. Once the process is completed, the officer will issue the certificate to the applicant within a day.

Through LifeSG app

1. The applicant can register a birth and apply for Baby Bonus at the same time using the Moments of Life (Families) app, can download the app for fee at IOS App Store or Android Google Play Store.
2. To use this app, the applicant should be:
   • At least one parent should be a Singapore citizen
   • Both parents’ have a SingPass account
   • Parent’s marriage is registered in Singapore at the time of child’s birth
   • The baby is born at KK Women’s and Children’s Hospital (KKH), Singapore General Hospital (SGH) and National University Hospital (NUH) and should have an electronic Notification of Live Birth (eNLB) number.
3. Login a app and follow the rules to register a birth.
4. After completion of registration, the applicant can get the certificate from the hospital where the baby was born, or at ICA registry of birth.

Required Documents

For registration

• Notification of Live Birth issued by the hospital/doctor/midwife
• The Identity Cards of the child’s parents
• The original marriage certificate of the child’s parents (if applicable)
• A letter of authorisation from the child’s parents (in case of registration by a proxy)

Additional original documents for a Foreigner:

• Passports of the child’s parents
• Long-Term Pass cards of the child’s parents
• Disembarkation/Embarkation cards of the child’s parents

Eligibility

• Singapore resident or a foreigner residing in Singapore are eligible to apply.

Fees

• S$18 for the Registry of Births & Deaths at ICA
• Fee is higher than S $18 and vary on each Birth registration centre

• Validity is forever.

• Birth Certificate is a vital document at various stages in life.

Information which might help

• Parents may apply to alter their child's given name before the child turns one-year-old, provided that a birth extract has not been issued before.
• A child's surname is declared and registered at the time of birth registration, if any, will remain unchanged.
• A birth extract, and not a new birth certificate, will be issued for any change of name.

Other uses of the Document/Certificate

• Other uses for travel, passport, proof of citizenship, social security, driver's license, school registration, personal identification and other legal purposes.
