Procedure

This procedure explains various ways to Obtain a copy of Birth Certificate in CALABARZON.

Apply In Person

1. Following are procedures on how you can obtain an authenticated copy of a birth certificate from the Philippine Statistics Authority (PSA) in person. You can request a copy of the certificate from any PSA Serbilis Center. To find the nearest one in your area click this - LINK.
2. Do not forget to bring a valid ID, and in case you are requesting a certificate for somebody else (not directly related) you will also need to present an authorization letter.
3. Fill up a request form.
4. Submit the form and pay the certificate fee.
5. Your request will be processed immediately. You will be able to collect your certificate on the same day.

Apply Online

1. Here's how you can request for an authenticated copy of a birth certificate from the Philippine Statistics Authority (PSA) online. Before starting your application, make sure that you have all the information you will need to provide when filling up the online form (see 'Required Information' section).
2. To request a birth certificate online, click this link - Apply Online.
3. To start the application process click, “Step 1: Fill out online application form” button.
4. On the next page, click the “Click here to request now” button.
5. Next, the ‘Terms and Condition’ page will open. The page contains information about who may request for a certificate, order fulfillment, payment, and cancellation. Once you are done reading the text, click the “I Accept” button.
6. Complete the Contact and Delivery Information form. After completing the form, click the “Next” button to proceed to the next step.
7. A confirmation page will appear. Review all information stated on the form and when you are done, click the “Confirm” button.
8. You will be taken to the request page. Click the “Birth Certificate” button.
9. You will need to answer questions 1 and 2 by clicking the “Yes” or “No” buttons. When you are done, click the “Next” button.
10. Next, you will be asked to complete a birth certificate request form. After completing the form, click the “Save” button.
11. If you would like to request another type of certificate, you may do so. You will see on the summary page a list of all certificates that you have requested. At the bottom of the page, you will see the total amount of your transaction. When you are done with your requests, click the “Submit” button.
12. When you submit a request, you will be issued a Batch Request Number and, for each request, a Request Reference Number. When paying for all requests by batch, specify the Batch Request Number. When paying for a particular request just, specify the Request Reference Number.
13. Pay for the certificate. To know how you can pay for your certificate click this button, “Step 2 - Pay through accredited payment channels”. This page contains information on how you can pay for your certificate, how much are the processing fee, delivery, and government taxes.
14. After your payment is confirmed, all you need to do is wait for your certificate to be delivered.

Required Documents

1. Valid government ID of the applicant
2. You will need to present an authorization and a valid ID of the birth certificate owner if you are requesting a birth certificate and you are NOT any of the following:
   • the owner of the birth certificate
   • his/her parent
   • his/her spouse
   • his/her direct descendant
   • his/her legal guardian or institution-in-charge, if minor

Delivery of the certificate: If you are the owner of the certificate and you are unable to receive the document, you may authorize a representative to do so on your behalf. Your authorized representative will have to present the following to the courier:

• your signed authorization letter
• your valid ID (with photo and signature)
• the valid ID (with photo and signature) of your authorized representative

Eligibility

Civil Registry documents can only be issued to the following:

• The owner of the certificate or his/her authorized representative
• The owner's spouse, parent, direct descendants, guardian or institution legally-in-charge of him/her, if minor
• The court or proper public official whenever absolutely necessary in administrative, judicial or other proceedings to determine the identity of a person
• In case of the person's death, the nearest of kin

Fees

PSA Serbilis

• Delivery within the Philippines: PHP 330.00 per copy
• Delivery to other countries: USD 20.30 per copy

Note: Fees are inclusive of processing, delivery and government taxes.

Validity

Birth Certificate is valid forever.
