Philippines - Online Apply Passport

STEP 1: Here are the procedures on how you can apply for a Philippine Passport online. First, you will need to book an appointment using DFA’s Passport Appointment System. To know which document you will need to submit when you apply for a passport, please see this link – Required Documents – Obtain a Philippines Passport.
STEP 2: Click this link - DFA Passport Appointment System – Apply Online. On the upper part of the page look for the “Schedule an Appointment” menu (Highlighted in RED). Click it to start the booking process.

STEP 3: A pop-up containing few reminders will appear. Read the reminders and once you are done, click the “START APPOINTMENT” button (Highlighted in RED).

STEP 4: Read the Terms and Conditions page. After reading it, tick the little box encircled in GREEN. Then proceed with the booking process by clicking the “START INDIVIDUAL APPOINTMENT” button (Highlighted in BLUE). NOTE: Option to book an appointment for a group is also available.

STEP 5: Another pop-up containing reminders will appear. To close the pop-up just click the “OK” button (Highlighted in RED) to proceed.

STEP 6: On the appointment page, you will be asked to indicate where you would like to process your application. Make use of the drop down menus (Encircled in RED) to select region and country. For applicants in the Philippines, you don’t need to change anything as the default setting is already set locally (DFA sites within the Philippines).

STEP 7: Next, indicate the specific DFA site where you would like to process your application. Use the drop down menu (Encircled in RED) and scroll through different DFA offices.

STEP 8: After selecting a DFA site, click the “NEXT” button (Highlighted in RED).

STEP 9: Next, you will have to set a date and time for your appointment. If you notice, the calendar is set to show the current month and year. In case you would like to apply for a different month or a different year, all you have to do is click the area highlighted in ORANGE.

STEP 10: Now, you can see other months with available slots (Highlighted in ORANGE). Click on the month of your choice.

STEP 11: Take a look at the calendar below (Highlighted in ORANGE). Click on the date of your choice. Note: Dates written in GREEN means there are available slots for that day.

STEP 12: The highlighted portion in ORANGE contains different available time slots. Just click on the time slot of your choice.

STEP 13: Once you have set a date and time for your appointment, tick the captcha box (Highlighted in ORANGE) and then click the “NEXT” button (Highlighted in BLACK box).

STEP 14: Complete the personal information form.

STEP 15: When you are done with the form, click the “NEXT” button (Highlighted in RED).

STEP 16: Next, you will need to fill up the Family Information Form. Once completed, click the “NEXT” button (Highlighted in RED box).

STEP 17: On the next page, you will be asked what type of application you are applying for. Select the application type as “New” from the drop down menu (Highlighted in ORANGE). Fill up the rest of the form and then once you are done, click the “NEXT” button (Highlighted in GREEN).

STEP 18: After entering your contact information, click the “NEXT” button (Highlighted in RED).

STEP 19: This is the Information Review page. Go over your application once more before finally submitting it for processing. Make sure that you double check everything (specially your name) because the DFA will exactly use the data that you have encoded when they process and print your passport.

STEP 20: Once you are done reviewing your application, tick the boxes encircled in GREEN and PURPLE below. And then, click the “CONFIRM” button (Highlighted with BLACK box) to finally submit your application to the DFA.

STEP 21: DFA will email you a copy of the completed form. Print and bring that form with you on the day of your appointment along with all other required documents.
STEP 22: On the day of your appointment, submit the printed application form to the front desk together with the required documents.
STEP 23: Pay the passport application fee.
STEP 24: Have your photo taken and your digital signature and fingerprints collected.
STEP 25: You can personally pick up your new passport once it’s ready. Or, you can also have it delivered to your home (optional) way.
