Procedure

Apply Online:

1. To Apply for SHS Voucher Program Online, the applicant has to make the application online via the following link
2. The applicant has to click on “Sign In” and enter their email and password to login.
3. The applicant has to fill in the electronic Voucher Application Form (VAF-1). VAs may do this in parts but must ensure changes are saved by section.
4. Make sure that the applicant has to upload all the necessary documents that are mentioned in the “Required Documents” section of this page.
5. Click the attestation button and submit the application.
6. An acknowledgment receipt will be provided. Please save it for future reference.
7. Internal and external evaluation will be conducted regarding the application.
8. The applicant can check the results through OVAP website.
9. There is no fee for this procedure is free of cost.
10. VAs with successful applications become eligible for the voucher and shall be called Qualified Voucher Applicants (QVAs).
11. Download the QVA Certificate from OVAP. The QVA Certificate is required for voucher redemption. QVAs shall submit their QVA Certificate to the SHS they decide to enroll at as proof that they are entitled to the voucher.
12. This entire process can be completed within 15 days or as per the timeline advised by the respective department authority.

• Recent 2x2 coloured ID photo
• Notarized Affidavit of occupation and income for ALL that apply: both parents, guardian/s, and other person/s helping send the VA to school, if any
• Signed Parent Consent Form for VAs below 18 years old at the time of the submission of the application
• Certificate of Financial Assistance received from the JHS, if applicable

Eligibility

• ll SY 2021-2022 Grade 10 completers in Private Schools who are not ESC grantees.
• All Grade 10 completers who completed Grade 10 prior to SY 2021-2022 but not earlier than 2016 and had not previously enrolled for Grade 11.
• Learners who had passed the ALS A&E Test for Grade 10 not earlier than 2016 and had not previously enrolled for Grade 11, or ALS learners who passed or will take the Portfolio Assessment in SY 2022-2023.
• Learners who passed the PEPT for Grade 10 not earlier than 2016 and had not previously enrolled for Grade 11, or learners who will take the PEPT for Grade 10 in SY 2022 - 2023.

Fees

• The fee for this procedure is free of cost.
