Procedure

Apply In-Person:

1. To obtain a death certificate for the animal, the applicant must approach the Veterinary Hospital of their respective area. The address and contact details of all the Veterinary Hospitals of Dadra and Nagar Haveli and Daman & diu can be found in this Link
2. Applicant must register the death of the animal by filling the prescribed form.
3. Collect the applicable application form from the office.
4. Fill the application form with the required information.
5. Attach the required documents as per details given in the “Required Documents” section of this page.
6. Submit the application form at the Veterinary Hospital office and make the required payment.
7. Death Certificate is then issued after proper verification.

Eligibility

Fees

• The death certificate of Animal is valid forever.
