ProcedureEdit

  1. Duplicates of a death certificate may be issued in case of loss, theft or destruction. They may also be issued in case of deterioration or were the certificate was incorrectly completed.
  2. Submit your request to a obtain a copy of the certificate at your local civil registry where the event was registered.
  3. Do not forget to bring a valid identification, you may be asked to present this to confirm your identity and to make sure that you have the right to obtain the certificate.
  4. If possible, you may also submit a copy of the civil status certificate lost, destroyed or stolen.
  5. Civil status certificates drawn incorrectly, damaged or withdrawn from use, must be surrendered for cancellation.
  6. Pay the application fee and wait for instructions from the Registrar regarding when can you comeback to collect the certificate.
