ProcedureEdit

  1. To replace a lost or stolen or passport, you will need to complete and submit an application to the FSM Passport Office. A link is provided under 'Documents to Use' section which you can use to download the form.
  2. The Passport Application Form must be filled out completely, signed and notarize prior to submission to the Passport Office.
  3. Submit or send the completed form and supporting documents to the FSM Passport Office (address provided below).


If you are outside Micronesia

You may use the link provided under "Office Locations & Contacts" to get in touch with the nearest Embassy of the Federated States of Micronesia in the country you are currently staying. The Embassy's consular services section will be able to help process your application to replace a lost or stolen passport.

Required DocumentsEdit

  • Completed application (The Passport Application Form must be Notarized)
  • Sworn affidavit detailing the circumstances surrounding the loss or theft of the passport
  • Two Identical passport photos of the applicant (size: 1 3/16 x 1 passport photo)
  • Payment of passport replacement fee payable to the FSM National Treasury

  • Lost/damaged/mutilated UNEXPIRED Passport: $75.00
