Procedure

• Citizens and residents of Qatar may apply for a health card to access the services provided by any of the public healthcare facilities or hospitals in the country.

Apply In-Person

1. To apply for the health card. The applicant should visit the nearest Primary Health Care Centre (PHCC) nearest to where you live. Locations of PHCC can be obtained from the following link to find out.
2. There you need to fill out the appropriate forms and submit it with all the required documents mentioned in the “Required Documents” Section of this page. These include:-

   • Passport or ID copy
   • Passport size photographs (two)
   • Residency permit (RP)
   • Qatar ID (issued when you get your RP)
   • A copy of utility bill (water, electricity or phone)
   • Vaccination card and birth certificate for children.

3. The health card application along with your documents can be submitted only from 7.30am until 2pm on weekdays to the PHCC offices.
4. The PHCC officials will go through your application and after verification you will be issued with your PHCC file number.
5. Make the appropriate payment via debit/credit card. No other form of payment is accepted. If you don’t have your own, you can bring a friend or relative along and use theirs.
6. PHCC can give the card the same day or will ask you to come any other day to collect the card.

Required Documents

Bring the following documents and pay appropriate fees:

1. Qatari Adults: Copies of your passport or ID, passport-size photo (4cm X 3cm).
2. Qatari Children: Birth certificate with Qatar personal identity number or passport, passport-size photograph and vaccination card (for first time issue).
3. GCC National Adult Residents: Copy of passport or ID with Qatar personal identity number, and passport-size photograph
4. GCC National Child Residents: Birth certificate or passport with Qatar personal identity number, one passport-size photo, and vaccination card (for first time issue for children born in Qatar)
5. Adult Residents: Valid QID or passport with valid residence permit and passport-size photo.
6. Children Residents: Passport with valid residence permit, passport-size photo and vaccination card (for first time issue and for those born in Qatar or who had their vaccinations in Qatar).
7. Domestic Staff: Valid passport with valid residence permit, passport-size photo and original QID of sponsor.
8. All: Copy of utility bill (phone, water/electricity) to prove where you live.

Eligibility

• Qatari citizens, GCC residents, non-GCC residents, visitors and domestic workers of Qataris are eligible to make this application.

Fees

• A fee of QR50 applies except in case of non-GCC adult residents and their children, the fees are QR100.
• In case of domestic staff, fees are upon the professions approved by the Ministry of Interior (MOI) for personal sponsorship.

• The card is valid for a period of one year.

Need for the Document

• Healthcare is provided for free for all citizens and residents of the country via Hamad Medical Corporation to individuals who have the health card.
• In the absence of a good health insurance plan, you will need to turn to Hamad General Hospital and Primary Healthcare Centres (PHCC) for all your medical needs.
• From consultations and blood tests to surgeries and medication, including for mental health, everything is free of charge or heavily subsidised in Hamad and PHCC. The Hamad health card will have your ID number, nationality, photo and date of issue.
• In order to access these services from Hamad Medical Corporation, you’ll need a health card

Information which might help

• You can also apply for a health card for domestic workers (maids, nannies, etc.) under your sponsorship. The application form is the same and you will need a copy of their Qatar ID. However, you must present the health card of the primary applicant (i.e. the person who sponsors the family) to verify the appropriate health center location. It is important to note that a water and electricity bill cannot be used for this purpose. The fee for domestic helper health cards is QR50.
