A Birth Certificate can be obtained from the Vital Statistics Office of the appropriate locality. If the birth occurred in a village or other place in which no vital statistics office is maintained, the record can be obtained from the Communal Vital Statistics Office (Urzad Stanu Cywilnego Gminnej Rady Narodowej) established for the community. Copies of the records can also be obtained from the district court having jurisdiction over the area, as duplicate copies of vital statistic records must be maintained as a part of such courts.
Apply in Person
- Prepare the necessary documents as mentioned in the documents section below.
- Visit any USC in Poland to submit your application for either.
- PLN 22 - for a short-cut copy(also for a multi-lingual copy)
- PLN 33 - for a complete copy
- Pay for the requested application you can:
- make a transfer to the account of the office where you submit the application,
- pay at the box office of the office where you submit the application and keep proof of payment.
- Submit the application with your documents for verification
- After verification, your application will be processed and issued.
Apply through Online

- Log in to your trusted profile.
- Complete the application.
- Sign the application with a trusted profile or qualified certificate(if you have one).
- Send the application. A message will be displayed that the application has been sent. You will receive an official submission certificate(UPP).
The service is not-localized - that is, you can send an application to any civil registry office.
If you want to apply online-you need a trusted profile (eGo). It allows you to confirm your identity. The point is that no one can impersonate you. check how to set up trusted profile
Apply through mail
- Gather all the documents required for the application and pay for the application fee.
- Send your application to USC in Poland through the contacts provided below in office and contacts.

- Visit the nearest embassy to submit your application or register in the systems e-konsulat
Apply through a proxy
- You can choose a representative to apply for the certificate as long as he/she has the power off attorney or is a legal representative.

- A person to whom the certificate refers to, or his children, parents, siblings, spouse or legal representative,
- Other persons who can prove a legal interest,
- Social organization, when justified by its statute or in favour of the public interest,
- Other interested parties - in the case of certificates of loss or destruction of the status book.

Fees are payable:
- Make a transfer to the account of the office where you submit the application.
- Pay at the box office of the office where you submit the application.
In the application you can use online payment. The fee for a copy of the act will then be increased by the cost of the transfer. Check if the office you are applying for accepts online payments. The list of municipalities that use online payments can be found on the Paybynet website of the National Clearing House .

Processing TimeEdit
You will receive a copy of:
- Immediately - if the file is in the electronic central register of civil status.
The files are prepared in the register from March 1, 2015. Acts made in paper books before that date must be transferred to it. Therefore, it may take longer.
- Within 7 business days from the date on which you submit the application - if you submit it in the Civil Registry Office, in which there is a paper file, but it has not yet been moved to the register,
- Within 10 business days from the day on which you submit an application - if you submit it in the Civil Registry Office, which does not have an act in paper form and has not yet been transferred to the register.

- Short extracts: birth certificate, birth certificate of a child who was born dead, marriage certificate, death certificate.
- Multilingual short extracts: birth certificate, marriage certificate, death certificate.
- A multilingual copy of a civil status document may be needed when you deal with matters abroad. It does not require translation.
- Remember that a short copy of the civil status file shows the current content of the act. It takes into account the changes that have been made in the act.
- Complete extracts: birth certificate, marriage certificate, death certificate, death certificate of a person of unidentified identity.
- Remember that a full copy of the civil registry file contains more information than a shortened extract - it is an exact repetition of the civil status file at the time it was made, along with the contents of later entries (changes).
You can get a copy of the civil status document shortened and complete in the form of your choice:
- paper.
- Electronic - an electronic document signed with a qualified electronic signature that confirms its authenticity. If you submit an application for a copy in an electronic form - you will get it to your inbox on the ePUAP platform (if you have an account there).
- Note: You can only get multilingual short-cut copies in paper form.
Required InformationEdit
- Civil status certificate id number and the name of Civil Registry branch office
- Personal details of the person involved/ child
- Date and place of birth
- Parents first and last names
- In case of confessional registration (in force until the end of 1945) religious congregation to which that person belonged, in which his/her birth took place.
Birth records contain data on the parents' age, profession, social standing, full place of residence, full date of birth and registration (down to the hour), and witnesses' ages, professions, and places of residence.

This document is used to prove citizenship of an individual and use in application of important documents like Identity cards and passports.
Information which might helpEdit
- The head of the Civil Registry Office may refuse to issue a copy of a civil status certificate if, for example, you want to get a copy of another person's act, but you do not have the right to do so. You will then receive an administrative decision on the refusal. If the head of the USC issues you with such a decision, you have 14 days to appeal to the voided who is subject to the Civil Registry. Cancellation of deposits via the head of the Tax Office that issued the decision.
Other uses of the Document/CertificateEdit
- The birth certificate is one of the documents that legally and unambiguously states who the parents are and so if one can prove one of the parents are Polish one can possibly apply for Polish citizenship.
- To apply for the Polish Passport, it is required to submit a Polish Birth Certificate.
- This document can be used to prove your age and kinship.

Who can receive a copy of the civil status certificate?
- The person whom the act concerns, and
- Her husband
- Another person from a close family - for example a parent, grandfather, grandmother, child, grandson, great-grandchild, siblings
- Statutory representative (e.g. parent) or guardian
- A person who proves a legal interest in obtaining a copy (that is, a legitimate interest which results from legal provisions, and presents relevant documents - for example, a court's obligation)
- Court
- Public prosecutor
- Social organization, if it justifies it with its statutory goals and speaks in favour of social interest
- A public administration institution (for example, a commune office), if it is necessary for the implementation of its statutory tasks.
Note: You can submit an application for a copy of a civil status record in person or through a proxy.
