• Visit the concerned authority.
• Fill the application form with the complete information.
• Submit the necessary documents.

• Original and copy of the Identity card or passport of adoptive parent or law firm or representative of a law firm
• Original appointment letter (law firm only)
• Original and copy of the Childs Adoption Certificate (AM233) if application is for replacement of certificate
• Statutory declaration SA81 if the original copy of the Adoption Certificate (AM233) is lost

Application for search or extract of the adoption register may be made by the following persons or entities:

• Owner of the Adoption Certificate
• Adoptive parent
• Adoptive sibling
• Law firm or its representative

Fees

The processing fee for this service is RM 5.00

Validity

Explain the time until which the certificate/document is valid.
e.g. Birth Certificate Valid Forever
