Procedure

• Verification or confirming of death is the process of identifying that a person is deceased. Immigration Department, National Registration Bureau, Pension Department, Foreign Missions, Employers, or Insurance companies may apply for Authenticating/confirming death certificates presented to them. Apply in Person:

1. To make the request visit the Department of Civil Registration or the local district registrar’s office, where the death occurred/ was registered as mentioned on the death certificate, and submit the supporting documentation as listed under the “Required Documents” section below to the register at the death registration section.
2. It should be noted that the letter requesting the service should be formal and should mention the following particulars precisely;
3. Full legal names of the applicant/organization/company.
4. Contact details of the applicant (Official Telephone number/ Email address/ Fax number/ Postal address).
5. Reason for the request.
6. Details of the Deceased (Full legal name, Identity card/ or birth certificate number, place, and date of death).
7. Declaration by the applicant (Company seal/stamp and signed by an authorized official).
8. Upon receipt of the application dossier, the registrar shall proceed and register the request the is no fees associated with this service.
9. The time frame for processing the application is within 14 days.
10. Upon completion of the search through the government vital record, where the death was registered the result will be obtained and shall be communicated to the applicant through the applicable channel.

• This is the process of confirming whether a person is actually deceased.
• The importance of confirming/verifying a death by government bodies or private companies e.g Insurance Companies is to;
• Confirmation of a death protects insurance companies against fraud in cases where the deceased had a life insurance policy.
• Through this process the Pension Department (NSSF) can confirm that its member has passed on and can now proceed with the payment of the benefits to the next of kin.
• This process assists the Immigration Department, National Registration Bureau, and Foreign Missions during the registration or application of identification documents such as national identity cards, passports, visas, etc.
