Procedure

Apply Online

1. To obtain a birth certificate, the applicant will have to visit the official Saskatchewan Vital Statistics website, use the following Website.
2. Enter the captcha and click on the type of certificate, in this case, it will be “Birth”.
3. Click on “Next”.
4. Enter the applicant’s details such as their name, sex and other details and click on next.
5. Once all the details have been entered, select the type of birth certificate required, and checkout.
6. Enter card details to make payment.
7. Submit the application by clicking on “Submit”.
8. The applicant shall receive their certificate by mail within six weeks from the submission of the application.

Apply by Fax

1. To obtain a birth certificate, download and fill in the application form, use the following Application Form.
2. Complete the application form with relevant details.
3. Call the Fax number: 306-787-8951
4. The applicant will have to send documents as mentioned under the “Required Documents” section of this page.
5. The applicant will have to make a fee payment, which is mentioned under the “Fees” section of this page.
6. The applicant shall receive their certificate by mail within six weeks front the submission of the application.

Eligibility

• The applicant should be applying for their own birth certificate.
• The applicant can be a parent of the person who is the subject of the certificate if that person is under 19.
• The applicant should be a custodial guardian if the parent is incapable, and the subject of the birth certificate is under 19.
• The applicants birth should have been registered in Saskatchewan.

Validity

• A birth certificate is valid forever and does not need to be renewed.

• The short-form copy of a birth certificate includes;
  • Name of individual
  • Date of birth
  • Place of birth
  • Sex
  • Registration date
  • Registration number, and
  • Date issued.
• The long-form includes;
  • Name of individual
  • Date of birth
  • Place of birth
  • Sex
  • Registration date
  • Registration number
  • Date issued, plus
  • The names, and the province or country of birth, of the parents.
