- To obtain a birth certificate, the applicant will have to visit the official Saskatchewan Vital Statistics website, use the following Website.
- Enter the captcha and click on the type of certificate, in this case, it will be “Birth”.
- Click on “Next”.
- Enter the applicant’s details such as their name, sex and other details and click on next.
- Once all the details have been entered, select the type of birth certificate required, and checkout.
- Enter card details to make payment.
- Submit the application by clicking on “Submit”.
- The applicant shall receive their certificate by mail within six weeks from the submission of the application.
Apply by Fax
- To obtain a birth certificate, download and fill in the application form, use the following Application Form.
- Complete the application form with relevant details.
- Call the Fax number: 306-787-8951
- The applicant will have to send documents as mentioned under the “Required Documents” section of this page.
- The applicant will have to make a fee payment, which is mentioned under the “Fees” section of this page.
- The applicant shall receive their certificate by mail within six weeks front the submission of the application.

EligibilityEdit
- The applicant should be applying for their own birth certificate.
- The applicant can be a parent of the person who is the subject of the certificate if that person is under 19.
- The applicant should be a custodial guardian if the parent is incapable, and the subject of the birth certificate is under 19.
- The applicants birth should have been registered in Saskatchewan.

The applicant can order 2 types of birth certificates:
- The short-form copy of a birth certificate includes;
- Name of individual
- Date of birth
- Place of birth
- Sex
- Registration date
- Registration number, and
- Date issued.
- The long-form includes;
- Name of individual
- Date of birth
- Place of birth
- Sex
- Registration date
- Registration number
- Date issued, plus
- The names, and the province or country of birth, of the parents.
