HOW TO APPLY 
#An applicant should visit the National Identification Registration Authority (NIRA) with all the required documents and bank slip for the application fees

==Instructions==
*A notification Record for a death that has occurred in a medical facility shall include a document duly signed and stamped by the Hospital- administrator or superintendent. 
*For a death that has occurred in the community, a death notification Record shall include a document signed and stamped by the Division – Town Clerk, Sub county – Sub county Chief, and shall have the following particulars of the event:-
:*Date of death.
:*Time of death.
:*Name of Deceased.
:*Place of Death.
*Once the notification documents are verified to be true the authority will register the death of the deceased person and consequently issue a death certificate signed by the notifier of a deceased’s death.
