*This application form is common for Mortgage, Transfer of Mortgage and Discharge of Mortgage, so the applicant must check the “Transfer of Mortgage” box at the beginning of the application form.

*It is sufficient for the applicant to complete sections B and C of the form. Application form can be typed or completed neatly by hand.

*It is important that the completed form must contain all the information required properly and the form must be filled legibly, in order to get accepted by the Authority.

*In case of more than one ownerships (Individual or joint), each owner must complete a separate application form.
